Objective medical decision~making has shown itself to be an emerging discipline which is sufficiently robust to promote its further development. This book identifies many important areas for applications in the field of acute patient care. The different approaches require testing, evaluation and mutual co~parisons to ensure that the right method is used to solve the existing problem. Medical sciences and patient care are increasingly supported by system sciences, resulting in growing multi- and interdisciplinary research and development areas. In this context, system sciences involve the methods, techniques, concepts and approaches obtained f~om disciplines such as mathematics, statistics, stochastic signal theory, fuzzy set theory, systems and control theory, signal analysis, pattern recogni tion, simulation, computer languages, structured programming, data base management and computer sciences. This book contains the papers of a workshop "Objective Medical Decision making f Systems approach in acute disease" which was initia ted and supported by the SWG/COMAC on Biomedical Engineering, Evaluation of Technology, Transfer and Standardization of CRM/CREST of the European Community. In setting up the original programme we have been assisted by a group of experts and for their cooperation we are very gratef.ul to S. Dawids, Copenhagen E. Epple, Tlibingen ; J. Jones, Harrow; L. Lambotte, Brussels C. Marchesi, Pisa and D. Robert, Lyon. The papers have been arranged in four groups, each followed by a brief synopsis. The four groups are : diagnosis, monitoring, therapy and control, and evaluation of criteria and procedures.

his volume of the series Lecture Notes in Medical Informatics contains the T proceedings of the Workshop on System Engineering in Medicine, which was held in Maastricht, The Netherlands, 16-18 March 1989. This workshop was sponsored by the EC under the framework of the Medical and Health Research Programme. The aim of the workshop was to assess whether there was sufficient support in the Medical Informatics community in the EC to establish a concerted action. This proceedings contain papers of the presentations given at the workshop. These presentations were centred around three themes: • Methods and Tools • Applications in the domains of chronic care and critical care • Evaluation of decision support systems The papers were prepared after the workshop and therefore we were able to include the relevant parts of the discussions which were related to the presentations. As a result of the discussions during the workshop, a proposal was prepared for the establishment of a concerted action, specifically addressing the development of guidelines for the evaluation of medical decision aids. This proposal was granted early 1990 under the same Medical and Health Research programme of the EC. Over 40 institutes are participating in this concerted action. It have been the outstanding presentations and the open discussions at the workshop that have been the starting point of this concerted action. The papers in this proceedings formed a starting point for the discussions in the meetings of the concerted action.

The 33rd Annual Meeting of the German Association for Medical Documentation, Informatics and Statistics was combined with a Special Topic Conference of the European Federation for Medical Informatics and takes place at Hannover, F. R. of Germany, from September 26 to 29, 1988. It was planned and initililly prepared by the late Prof. P. L Reichertz, who headed the Hannover institute from 1969 to 1987. To commemorate his contribution to the development of medicine the conference was devoted to him "Peter Reichertz Memorial Conference on Expert Systems and Decision Support in Medicine" Since computers in the early Fifties were first applied to support medical reasoning, various phases of euphoria and resi~ation have . followed. Every new methodology which became technically possible was and will be applied to the old questlon of how to diagnose diseases more reliably. Artificial Intelligence is just one new approach to the old challenge. Over the years some· authors have been very optimistic and put forward opinions which motivated the common press to coin the phrase 'Dr. med. computer'. Papers printed under this heading rebuffed the majority of physiCians for many years. Today we know that medical decision making is a most complex buman performance. And 30 years of research on decision support have given us only limited insight into the underlying processes. Most of the principal methodological questions were already asked very early on.

Psychoanalysis and information science are two disciplines that twenty years ago hardly anyone would have considered closely related with re gard either to persons or subject matter. The cooperation between the two fields was made possible, on the one hand, by the development of information science from a technological discipline to a theoretically founded scientific discipline, and on the other, by the development of psychoanalysis from a hermetically closed special discipline to an open and empirical science. This study reports the successful inclusion of information science as an instrument of research on the psychoanalytic process. This direction of work was initiated by Donald Spence and Hartvig Dahl from the Re search Institute of Mental Health, New York, at the end of the 1960s. H. Thom~, H. K~che1e, and their research group in Ulm must be credited with having adopted Spence's programs as early as 1974; at that time the programs even had to be implemented far away on the IBM computer at the University of Heidelberg. The efforts to expand computer-aided con tent analysis to a method for process research led in 1975 to the ac quisition of the Hamburg program for Electronic Verbal Analysis (EVA). In the following years the author of this study revised, extended, and supplemented the EVA to create the U1m version. This book now describes another step in the integration of information technology in fundamen tal research in psychoanalysis.
