Le scienze del comportamento e le neuroscienze hanno avuto un enorme crescita negli ultimi decenni tanto da portare ad una vera e propria rivoluzione, non solo in campo scientifico ma anche nelle abitudini delle persone e negli stili di vita. Ne sono prova, da un lato, l’enorme proliferare di riviste scientifiche e la percentuale di articoli pubblicati sul totale delle varie discipline, ma anche l’aumento esponenziale delle rubriche psicologiche e psichiatriche su qualsiasi rotocalco, rivista divulgativa, programma televisivo ecc. In altri termini, è sicuramente possibile affermare che la psichiatria, la psicologia, la psicoterapia, da discipline che ingeneravano sospetto e timore, stanno divenendo fonte di curiosità, di attenzione con un sempre maggiore “appeal” sulla popolazione. A questo fenomeno ha fatto riscontro anche un aumento numerico degli operatori del settore che, già di per sé variegato, ha avuto grossi problemi di comunicazione, essendo molti gli approcci e le “teorie” prese come punto di riferimento dai vari clinici. L’importante evento degli ultimi 20 anni, è rappresentato dalla nascita e sviluppo di guide cliniche per la diagnosi categoriale quali l’attuale DSM V 1 e l’ICD 102, ottimi ed insostituibili riferimenti per il clinico e che hanno consentito, per quanto riguarda le malattie mentali ed i disturbi del comportamento umano, una notevole omogenizzatine dei linguaggi e degli stili di approccio al problema. Come tutte le iniziative umane, se questo ha portato ad un enorme beneficio in termini di chiarezza e facilitazione della comunicazione tra operatori, in taluni casi si è assistito alla sottovalutazione di alcune delle componenti essenziali nell’approccio clinico al problema, con una inevitabile preferenza degli aspetti più generali e statistici, appunto, rispetto ad altre componenti più soggettive e peculiari del singolo che più sono sondabili ed evidenziabili con un approccio più di tipo semeiotico dinamico. Questo testo nasce perciò dall’esigenza di fornire allo studente, ma anche allo specializzando ed all’operatore clinico, sia esso psichiatra, psicologo clinico, psicoterapeuta, o altro operatore della salute, una guida teorico - pratica alle componenti principali dell’attività del clinico: quella di studiare anche con metodi oggettivi il caso per arrivare a formulare una diagnosi funzionale (non solo di nome), al paziente, al problema ed al contesto nel quale ogni singolo individuo si trova calato. Il principale scopo del volume è perciò quello di poter offrire un ausilio per la diagnosi clinica che può essere anche, ma non esclusivamente, descrittiva in psichiatria e psicologia clinica ma non può esimersi dall’essere anche globale, multidimensionale, e, in molti casi, anche multidisciplinare. Vi è poi la necessità di acquisire conoscenze metodologiche per la verifica delle ipotesi diagnostiche al fine di una corretta presa in carico, per la proposta e prescrizione del migliori trattamenti possibili, sia farmacologici che psicologici nonché per la verifica, anche in itinere, della loro efficacia. Il testo va perciò inteso, come un ausilio per il clinico, sia esso psichiatra o psico- logo, come un’integrazione del DSM con la proposizione di approcci per l’approfondimento del caso. Nel testo si tratterà quindi del come ottimizzare un colloquio ed un’anamnesi mirata, della presentazione di alcuni test psicodiagnostici adeguati alle varie caratteristiche del problema presentato, di procedure di analisi strumentale psicofisiologica, di come utilizzare proficuamente dati provenienti da analisi di laboratorio medico. Si propongono inoltre linee guida in gran parte svincolate dall’approccio teorico preferito dall’operatore, in particolare dello psicoterapeuta, in modo che questo, indipendentemente dal fatto che sia ad indirizzo dinamico, cognitivo, biologico, ecc., possa poter fruire di protocolli adeguati per l’impostazione del trattamento più idoneo possibilmente in termini di terapia integrata e per la valutazione in itinere e nel follow up, della bontà e dell’efficacia dei trattamenti.
