This book reflects debates and results which have developed since the introduction of an international system of classifications in epileptology. The creation of such a system was initiated, in the sixties, mainly for practical reasons: growing international exchange had revealed that divergence of terminology in epilepsy had become important enough to prove a serious obstacle to sensible discussions. The Bethel-Cleveland symposia, which was at the origin of this book, aim to bring the excellence of advanced inter-disciplinary and controversial workshops to a larger public. The book concentrates on a selection of topics where progress has been made, where controversies are open or where discussion needs to be stimulated.

Excerpt from On Epilepsy and Epileptiform Seizures: Their Causes, Pathology, and Treatment The subject of epilepsy is one that embraces, perhaps more than any other limited department of medical science, the whole range of pathology and therapeutics. It is with this conviction that I have laboured at the changes which the present edition seemed to demand. I have sought in it to bring to bear not only my own matured experience, but also the advances in physiology and pathology with which the last few years have teemed. The first edition was brought out a little more than three years ago, under circumstances which might palliate certain crudities of composition. As, however, it met with a reception beyond its merits, I indulge in the hope that its successor, worked out with the same love, though with more care, may not prove un acceptable to my medical brethren. Although I have retained the mould in which the work was originally cast, I have, acting upon the advice of one well qualified to express an Opinion, omitted the summary of cases given in the former edition. Instead of it I have interspersed numerous.m'iii preface To the second edition. About the Publisher Forgotten Books publishes hundreds of thousands of rare and classic books. This new edition of the book series dedicated to “Progress in Epileptic Disorders” is the result of a recently held unique gathering of international experts that debated on the prognostic and therapeutic issues raised by the management of first unprovoked seizures and of newly diagnosed epilepsy. Current knowledge on natural evolution of a first seizure, the role of co-morbid conditions, the impact of immediate versus delayed treatment and recent guidelines are thoroughly addressed. The new ideas and suggestions that emerge from this book offer challenging perspectives for both patient care and clinical as well as fundamental research. The first section of the book includes an up to date review of epidemiological data, encompassing the natural evolution of single unprovoked seizures, risk factors for recurrent seizures, and the potential for comorbid conditions such as depression, migraine, ADHD, and dysimmune metabolic disorders to promote the development of epilepsy. The impact of febrile convulsions on long-term brain hyperexcitability is also discussed. The second section concentrates on therapeutic issues, concerning the management of single unprovoked seizures and newly diagnosed epilepsy. These include the impact of immediate versus deferred treatment and the choice of the most appropriate antiepileptic drug therapy. Several methodological issues are addressed in parallel with the chapters, providing new insights into domains, such as the representativeness of the patient population in the studies under consideration, as well as the limitations and pitfalls of most randomised controlled trials. The value of these trials in the development of treatment guidelines is also discussed.
