The Second Edinburgh International Workshop was held in September, 1984 and took as its topic the coordinated regulation of gene expression. The intention of this series of workshops is to promote exchange of ideas and data between scientists and clinicians whose interests span molecular and cell biology, development and differentiation, oncology, and genetic and developmental pathologies. It is hoped that such interdisciplinary discussions may give rise to fruitful insights. The meetings are structured to give ample time for discussion after each formal presentation and culminate in a session of general discussion which is reported at the end of the volume of proceedings. We are very grateful to the participants, all of whom participated in the discussion and whose contributions were essential to the success of the meeting. Novel ideas were often put forward and explored thoroughly from different angles. We normally expect to achieve quite rapid publication of the proceedings of the meeting and are grateful to authors who produced typescripts of their contributions expeditiously, but, as may sometimes be the case with multi-author works, some contributors had difficulty in meeting schedules for submitting manuscripts or corrections of the text of the discussion, and in one case we have been unable to publish any record of the contribution. Our committment to the publication of the discussion, allowing participants to make corrections to the transcript of the session, such as insertion of references and clarification of oral contributions, has also imposed some delay.

At the midpoint of the 20th century, our knowledge of cancer was based on epide- ology and pathology, and treatment consisted of surgery and radiation therapy. At mid-century, Medawar and colleagues initiated the understanding of transplantation immunology, Farber described the first use of an antifolic drug to treat leukemia, and Jacobson and coworkers described the irradiation-protection effect of spleen cells. These observations opened the door to the development of chemotherapy and tra- plantation in the treatment of cancer. Despite the rapid development of these new disciplines, progress was usually based on empiric observations and clinical trials. The rapid advances in molecular biology at the end of the 20th century mark a new era in our knowledge of cancer. Molecular immunology, molecular genetics, mole- lar pharmacology, and the Human Genome Project are in the process of providing a level of understanding of cancer undreamed of in the past. Optimism is based on the firm belief that understanding at the molecular level will lead to better and earlier di- nosis, to new forms of treatment, and, most importantly, eventually to prevention of many types of cancer.

MicroRNAs have recently emerged as key regulators of gene expression during development and are frequently misexpressed in human disease states, in particular cancer. These 22-nucleotide-long transcripts act to promote or repress cell proliferation, migration and apoptosis during development, all of which are processes that go awry in cancer. In this book, tumour growth is perceived as a deviation from the normal development of the human organism. The molecular, cellular, and tissue determinants of different tumours are discussed showing that each is a different disease, often corresponding to a particular developmental stage. The natural history of several cancers illustrates how clinical incidence can be just the visible part of the iceberg, while the first changes at the tissue level sometimes occur several years before tumour growth becomes manifest. Several mechanisms are proposed to explain the distribution of cancers during the human life span and the decline of the incidence of cancers during human senescence.
