The NAO examined waiting lists because although they are an important performance measure there has been considerable debate about what the statistics actually represent and what has been the impact of initiatives to reduce times. Additionally it was an opportunity to identify good practice. They found that the outpatient waiting list only measured those who had been waiting more than thirteen weeks to see a consultant for the first time (the timing of subsequent appointments was not measured but second or subsequent attendances represent 72 per cent of appointments). Inpatient waiting list reports the number of patients awaiting a first admission for an elective procedure under the care of a consultant. The report recommends that more attention should be focused on the total time patients' wait from GP referral to treatment. They also found there was variation and inconsistency in the way that the statistics were kept and recommend that the lists be verified every six months. Other recommendations were for research into why there is a wide variation in waiting times and a programme to implement best practice.

In 2005-06 some £259 million was spent on outpatient services in Northern Ireland. A perennial cause of inefficiency in the provision of these services has been patients' failure to attend appointments and the cancellation of clinics by Trusts. In September 2006 almost 15,000 consultant-led clinic sessions were cancelled and there were 200,000 cases of patients not attending. A survey found that some 10 per cent of outpatients are "not seen" through cancellation or non-attendance, an opportunity cost of £11.6 million. Health service managers and clinicians need to monitor non-attendance/cancellation rates closely, in order to clarify the factors influencing them, and to allow planning of effective strategies aimed at minimising any loss of time and resources. The report also notes that outpatient services run by healthcare professionals other than consultants are not covered in the weekly statistical management information currently collected: if these are included outpatient activity levels are twice that officially reported. Changes to the way in which patients and general practitioners access outpatient services are being implemented and they should reduce waiting times and have an impact on cancelled clinics and missed appointments.

This report by the Committee of Experts on criteria for the management of waiting lists and waiting times for health care focuses on waiting for hospital treatment, both out-patient and in-patient, and waiting times to the start of treatment or examination in hospitals. It gives a number of reasons for waiting times and waiting lists: the need for care has increased; growing expectation and demand; new technologies leading to wider indications for treatment and an increase in demand; managerial or administrative shortcomings; logistical problems; absence of data; decreasing resources; medical practice - priority setting. The report recommends that the governments of member states review the existence and role of waiting lists and waiting times and where necessary, develop comprehensive and coherent strategies for the management of waiting lists and waiting times. These strategies should take into account the criteria set out in the appendix to the recommendation and the principle of equity in access to health care. The report also recommends that governments ensure that the recommendation is brought to the attention of all relevant actors and adopt the legislative or other measures necessary to give effect to the principles set out in the appendix to the recommendation. Annexes give; examples of minimum data sets from Spain and the UK and suggested outlines for a hospital waiting list policy and for establishing health registers.
