The U.S.-Associated Pacific Basin consists of six island jurisdictions: American Samoa, the Commonwealth of the Northern Mariana Islands, Guam, the Federated States of Micronesia, Republic of the Marshall Islands, and the Republic of Palau. This book examines one aspect of the ties and U.S. involvement with this part of the world--its role in the region's health care delivery system. Although the health status of the islanders and the challenges faced by the health care systems naturally vary within and among the jurisdictions, in general, almost all health indicators for the islanders are worse than those of mainland Americans. The health systems in the area must deal with conditions normally seen in developing countries (e.g., malnutrition, tuberculosis, dengue fever, and cholera) and in developed countries alike (e.g., diabetes, heart disease, and cancer). In examining the strengths and weaknesses of the area's systems, the volume provides a regional health overview and assessments of health care in individual jurisdictions, evaluates the Pacific Basin Medical Officers Training Program, and lays out a strategic plan for future health services in the U.S.-Associated Pacific Basin.

The mega-regional agreement, the Trans-Pacific Partnership, put forward a radical model for the regulation of intellectual property and access to medicines across the Pacific Rim. The trade agreement makes reference to the framework established by the TRIPS Agreement 1994, the Doha Declaration on the TRIPS Agreement and Public Health 2001, and the WTO General Council Decision 2003 (which has been incorporated into the TRIPS Agreement 1994 as an amendment in 2017). Nonetheless, it does little to positively advance public health and access to medicines. The Trans-Pacific Partnership seeks to maximise the intellectual property rights of pharmaceutical drug companies. The agreement has extensive provisions on patentable subject matter, patent standards, patent term extensions and evergreening, patent registration linkages, and border measures. There has also been controversy over measures related to data protection, the protection of biologics, and trade secrets. The World Health Organization and the United Nations Secretary-General's High Level Panel on Access to Medicines have highlighted the need to ensure public health and access to medicines are not undercut by regional trade agreements, such as Trans-Pacific Partnership.

The Trans-Pacific Partnership (TPP) talks attempt to link together at least nine countries in three continents to create a 'high-quality, twenty-first century agreement'. Such an agreement is intended to open markets to competition between the partners more than ever before in sectors ranging from goods and services to investment, and includes rigorous rules in the fields of intellectual property, labour protection and environmental conservation. The TPP also aims to improve regulatory coherence, enhance production supply chains and help boost small and medium-sized enterprises. It could transform relations with regions such as Latin America, paving the way to an eventual Free Trade Area of the Asia Pacific, or see innovations translated into the global trade regulatory system operating under the WTO. However, given the tensions between strategic and economic concerns, the final deal could still collapse into something closer to a standard, 'twentieth-century' trade agreement.

The Trans-Pacific Partnership, with its twelve participating countries on three continents, is the largest regional trade and investment agreement that Canada has ever negotiated. It is also one of the most controversial— for good reason. Negotiations ended exactly a year ago, in October 2015, and the TPP was signed in New Zealand in February 2016. But there is no guarantee it will ever come into effect. Opposition to the TPP is strongest in the United States, where both 2016 presidential nominees vowed to kill or significantly renegotiate the deal. Outgoing President Barack Obama characterized the TPP as a Made-in-America deal in the hope of getting it passed into law shortly after the November presidential election. But is what is good for corporate America good for Canada? In this book, experts in a dozen policy areas explain what the impact of the TPP agreement would be on Canada. Many of the key issues they explore have received little media coverage, notably the effect of the TPP on environmental protection, health care and other public services, Canada's cultural industries, the labour market, human rights and the democratic decision-making process generally.
