Pain is a highly subjective experience and not necessarily related linearily to a nociceptive input. It is influenced by pathological and genetic factors, and is further subject to a complex set of individual, contextual, and environmental variables. Emotional states are thereby associated with an intricate interplay of coordinated neurotransmitter systems that lead to endogenous pain modulation. Mu-opioidergic neurotransmission in emotional pain modulation is one of the key aspects of this thesis. The findings clearly demonstrate that pain is a dynamic experience, by which noxious stimuli are permanently modulated and re-evaluated within a broader context of the individual and its interaction with the environment. The acknowledgment of the organism as a unity, where mental phenomena cannot be separated from their physical consequences, shapes our understanding of 'bodily' and 'emotional' homeostasis. It should therefore be of great value for both the pharmacological care of pain patients and the individual and clinical management of other life's challenges.

There is good evidence that placebo and nocebo responses reflect more than a psychologic reappraisal of pain perception. In fact, pain perception is the endpoint of several endogenous pain inhibitory and facilitatory mechanisms that trigger descending pain modulation mechanisms to the spinal cord. Measuring the ability of placebo and nocebo to act on these mechanisms will demonstrate that their effects are not only a reinterpretation of higher centers of the same nociceptive signal, but are acting at the source of this signal at the spinal level. This information is important considering that the capacity to trigger endogenous pain modulation mechanisms is a good predictor of pain chronification. This chapter addresses the importance of psychologic factors on endogenous pain facilitatory and inhibitory mechanisms in the central nervous system, from the cortex to the spinal cord, and their role in treatments targeting the same mechanisms.

Within the past few years, it has become recognized that the immune system communicates to the brain. Substances released from activated immune cells (cytokines) stimulate peripheral nerves, thereby signaling the brain and spinal cord that infection/inflammation has occurred. Additionally, peripheral infection/inflammation leads to de novo synthesis and release of cytokines within the brain and spinal cord. Thus, cytokines effect neural activation both peripherally and centrally. Through this communication pathway, cytokines such as interleukin-1, interleukin-6 and tumor necrosis factor markedly alter brain function, physiology and behavior. One important but underrecognized aspect of this communication is the dramatic impact that immune activation has on pain modulation. The purpose of this book is to examine, for the first time, immune-to-brain communication from the viewpoint of its effect on pain processing. It is aimed both at the basic scientist and health care providers, in order to clarify the major role that substances released by immune cells play in pain modulation. Placebo analgesia is an illustrative example of the impact that psychological factors can have on the experience of pain. While this form of pain modulation is mediated by multiple neurobiological mechanisms, one influential explanation posits that a descending pain control system contributes to placebo effects in pain. Here, we first give an overview of descending pain control as established in animal studies, focusing on an opioid-dependent system that includes the periaqueductal gray (PAG) and rostral ventromedial medulla (RVM) as core regions and controls nociceptive processing already at the level of the spinal cord. We then review functional magnetic resonance imaging (fMRI) and positron emission tomography (PET) studies that provide evidence for an involvement of this system in placebo analgesia. Finally, we have a look at the role of the spinal cord in placebo analgesia, focusing on spinal fMRI studies.
