Il campo delle perversioni sessuali è vasto e complesso, con differenti cause psicologiche e una varietà praticamente infinita di comportamenti. Aspetti parafilici possono manifestarsi in vario modo: sotto forma di fantasie, non sempre riconosciute o accettate; possono esprimersi in una modalità soft, sotto forma di gioco erotico, funzionale alle esigenze della coppia; possono manifestarsi in modo solitario e nascosto; oppure possono arrivare a gradi estremi, con comportamenti ad alto rischio, a volte della vita stessa. L’estrema diversità di espressione rende così le parafilie una dimensione psicopatologica molto ampia, con differenti significati e gradi di gravità. Le perversioni impongono la riflessione su cosa sia la normalità ma anche, e soprattutto, sul senso stesso della sessualità. In questi comportamenti il significato riproduttivo è perso definitivamente e si amplifica in modo abnorme una comunicazione distorta con l’altro e con se stessi. Questo avviene contemporaneamente ad un livello psichico e corporeo, con un fitto intreccio di aspetti mentali e comportamentali. Così l’indagine sulle perversioni fa emergere meccanismi psichici fondamentali, che sostengono la vita mentale di ogni essere umano. La parafilia utilizza l’energia sessuale, intesa non solo come pulsione, che qui spinge all’azione in modo aberrante. L’analisi e lo studio delle perversioni è un’occasione per far emergere aspetti e dinamiche nascoste, disagi, traumi, conflitti, difficoltà di comunicazione, di relazione, di rapporto con il proprio corpo e con quello altrui. L’attività sessuale diviene compulsione, spesso estrema e pericolosa, ove il piacere si lega al dolore, al pericolo e alla violenza. Il campo delle perversioni, affascinante e difficile, richiede uno studio su più fronti: psicologico, biologico, relazionale, esistenziale. Questo volume riprende una mia prima pubblicazione di circa 20 anni fa, ove cercavo di aprire una via di ricerca in questa area di contatto tra psichiatria e sessuologia. In questi anni molte cose sono cambiate nella medicina, nella psichiatria e nella nostra cultura, non solo in senso sessuale. La diffusione sempre maggiore di internet, con l’estensione abnorme di materiale porno, ha contribuito ad una sempre più accentuata distorsione del significato della sessualità. Si sono sviluppate nuove perversioni, nuove modalità, nuove possibilità, con il risultato di rendere sempre di più il sesso un gioco, una p re s t a z i o n e , u n a e s i b i z i o n e . La perversione è divenuta parte integrante e normale della vita sessuale, come per esempio nelle cosiddette perversioni soft, perdendo di vista il suo senso originario, di comunicazione affettiva oltreché riproduttiva. La sessualità è una funzione biologica fondamentale, è la base della vita, è ciò che consente la sopravvivenza di una specie, o anche solo di un individuo, come eredità genetica. Essa è una forza che spinge alla relazione, attraverso il corpo più che la mente. Nella comunicazione relazionale è facile che vi siano distorsioni, alterazioni o difficoltà, che trovano nelle parafilie un canale di espressione particolare e potente. L’analisi, da un punto di vista psicologico, psichiatrico e medico delle perversioni, proposto in questo libro, vuole essere un momento di riflessione che possa aprire a nuove ricerche, ancora scarse in questo settore, che consentano una migliore comprensione dell’uomo nelle sue più varie manifestazioni. E le parafilie sono sicuramente una di quelle più forti, complesse e misteriose.

Perversions and borderline states were, by accident of fate, Masud Khan's chief preoccupation in his clinical work during the last three decades of his life. Essentially clinical in its approach, Psychic Retreats discusses the problem of patients who are 'stuck' and with whom it is difficult to make meaningful contact. John Steiner, an experienced psychoanalyst, uses new developments in Kleinian theory to explain how this happens. He examines the way object relationships and defences can be organized into complex structures which lead to a personality and an analysis becoming rigid and stuck, with little opportunity for development or change. These systems of defences are pathological organisations of the personality: John Steiner describes them as 'psychic retreats', into which the patient can withdraw to avoid contact both with the analyst and with reality. To provide a background to these original and controversial concepts, the author builds on more established ideas such as Klein's distinction between the paranoid-schizoid and depressive positions, and briefly reviews previous work on pathological organizations of the personality. He illustrates his discussion with detailed clinical material, with examples of the way psychic retreats operate to provide a respite from both paranoid-schizoid and depressive anxieties. He looks at the way such organizations function as a defence against unbearable guilt and describes the mechanism by which fragmentation of the personality can be reversed so the lost parts of the self can be regained and reintegrated in to the personality. Psychic Retreats is written with the practising psychoanalysts and psychoanalytic psychotherapists in mind. The emphasis is therefore clinical throughout the book, which concludes with a chapter on the technical problems which arise in the treatment of such severely ill patients.
