CHARLES Y. c. PAK Major progress has been made in the pathophysiologic elucidation and management of nephrolithiasis during the past two decades. It is now possible to detect the cause of stone disease in more than 95% of patients, to prevent recurrent formation of stones in the majority of patients, and to remove most existing stones less invasively. The assumption of editorship of this book permits me to indulge in the discussion of this progress from my personal perspective. Three somewhat fortuitous events in my academic career dictated my directing major efforts in stone research. The first event occurred in 1963 when, after having completed medical training, I was faced with two years of military service as a participant of the Berry plan. Choices were limited and disconcerting for someone interested in a research career: a staff physician at a military installation or an indian reservation, or a member of a research team in a state penitentiary. An interesting article by Norman Gershfeld on phospholi pid monolayers prompted me to write him seeking a position in his laboratory of Health (NIH) in Bethesda, MD. Partly because of at the National Institutes my rudimentary exposure and publication in surface chemistry, I was offered a position as a staff scientist and a position in the Public Health Service which satisfied the requirements of a military service.

In recent decades, we have enhanced our understanding of the pathophysiology and genetics of rare and common causes of kidney stones. With our evolving understanding of the epidemiology, biology, and genetics of nephrolithiasis and the advances in therapeutic technologies, we have made significant progress in patient care. Furthermore, advances in the medical management and surgical technologies have allowed us to embellish the optimal outcomes in the management of complex kidney stone disease.

"People . . . see you sweat in agony, turn pale, turn red, trem ble, vomit your very blood, suffer strange contractions and con vulsions, sometimes shed great tears from your eyes, discharge thick, black and frightful urine, or have it stopped up by some sharp rough stone that cruelly pricks and flays the neck of your penis. " * These 16th century frustrations of Michel de Montaigne which most graphically reflect his experience with renal colic still plague approximately 1 per 1000 individuals in the United States annually. Since as many as 75% of clinically. apparent episodes of renal colic represent single nonrecurring events, physicians not in frequently approach the differential diagnosis of nephrolithiasis in a less than adequate fashion and assume that the incident may prob ably never recur after the single attack. However, the ureteral or bladder stone actually represents one form of abnormal crystalline precipitation; parenchymal nephrocalcinosis, silently progressive azotemia, and asymptomatic renal pelvic calculi may also stem from the same pathological process(es) which conditioned the for mation of the ureteral or bladder stone. In this regard it is worth emphasizing that the postmortem incidence of renal calculi is some tenfold greater than that presumed from surveys wherein a clinical attack of nephrolithiasis is the sole determinant. *In Familiar Medical Quotations. M. B. Strauss, Ed. Little Brown, Boston, 1968, p. 646. v vi FOREWORD The last decade has witnessed the birth of new knowledge in urolithiasis research.

Urolithiasis, or stone disease occurs in 7% of women and 12% of men at some point, and these statistics are rising. Furthermore, for those who form a stone, the likelihood of a recurrence is nearly 50% within 5 years of initial diagnosis. Therefore, the need for effective, minimally invasive alternatives for stone eradication and prevention is critical. Because stone disease comprises a large part of any urologist's practice, a thorough knowledge of the pathogenesis and pathophysiology of stone disease is critical in order to understand and implement treatment strategies to prevent stone formation. Likewise, a working knowledge of the surgical treatments, instrumentation and outcomes is necessary to arm patients with sufficient information to make an informed decision and to provide the appropriate treatment modality for a given stone situation. In Urolithiasis: Medical and Surgical Management, the authors provide a complete guide to the management of stone disease from both a medical and surgical prospective. This book should be an invaluable resource for those who treat stone disease in any capacity, whether surgically or medically, acutely or long-term.

Upper urinary tract stones are a significant source of morbidity and cost in industrialized society. With genetic, environmental and metabolic risk factors for stone formation, stone disease affects a large cross section of the population. Consequently, strategies aimed at stone prevention are attractive. To that end, a thoroughly understanding of the pathogenesis and pathophysiology of stone disease is critical in order to target key physiologic processes that can be manipulated in order to prevent stone formation. Likewise, the identification of individuals at high risk of stone formation can streamline the selection, evaluation and treatment of patients with a history of stones. However, despite our best efforts to prevent stone formation, stones do and will continue to occur. As such, surgical treatments with the least morbidity and highest effectiveness are continuously sought. In this issue of Urologic Clinics, all aspects of stone disease, from pathogenesis to surgical treatment to prevention is explored in order to provide the reader with a comprehensive understanding of stone disease and an update on the most recent innovations in surgical and medical management.
