This booklet which includes a CD-ROM should enable any health-care worker to facilitate a workshop on patient safety. This workshop explores how multiple weaknesses present within the hospital system can lead to error. It aims to provide all health-care workers and managers with an insight into the underlying causes of such events. Although the workshop materials revolve around an error involving the inappropriate administration of vincristine, the underlying principles of why an error occurs are universal and the learning objectives can be applied in any error-related situation.

Following recent high profile cases of surgical error in the UK and USA, patient safety has become a key issue in healthcare, now placed at heart of junior doctor's training. Errors made by doctors are very similar to those made in other high risk organisations, such as aviation, nuclear and petrochemical industries. Practical Patient Safety aims to demonstrate how core principles of safety from these industries can be applied in surgical and medical practice, in particular through training for health care professionals and healthcare managers. Whilst theoretical aspects of risk management form the backdrop, the book focuses on key techniques and principles of patient safety in a practical way, giving the reader practical advice on how to avoid personal errors, and more importantly how to start patient safety training within his or her department or hospital.

Over the last two decades across the globe we have seen a multitude of programs, projects and books to help improve the safety of patient care in healthcare. However, the full potential of these has not yet been reached. Most of the current approaches are top down, programmatic and target driven. These look at problems in isolation one harm at a time with simplistic solutions that fail to support a holistic, systematic approach. They are focused on collecting incident data and learning from failure using tools that are not fit for purpose in a complex nonlinear system. Very rarely do the solutions help build the conditions, cultures and behaviours that support a safer system and help the people involved work safely. Healthcare is stuck in a relentlessly negative approach to safety. Those working in patient safety and healthcare are struggling, and books on patient safety to date instruct the reader to continue doing the same things we have been doing for the last 20 years. This book uniquely combines the latest thinking in safety, including creating a balanced approach to learning from what works as a way to understand why it fails, together with the evidence on building a just culture, positive workplaces and working relationships that we now know are so important for safety. It helps people understand how to address issues despite their complexities and improve safety with practical ways to truly understand what day to day healthcare work is actually like, rather than what people imagine it is like. This book builds on the author’s first book Rethinking Patient Safety which exposed what we need to do differently to truly transform our approach to patient safety. It updates the reader further on the concepts explored in the first book but also vitally helps readers understand the ‘how’. Implementing Patient Safety goes beyond the rhetoric and provides the reader with ideas and examples for how the latest thinking can actually be achieved. It is based on the author’s personal experience of leading a national culture change campaign in the National Health Service for five years. The lessons arise from helping hundreds of organisations and people rethink and implement a whole new way of thinking about improving patient safety in healthcare.

Background: The principal component in up to 70% of the critical incidents in anaesthesia is human factor. Among the different tools to promote patient safety, we have instituted a multidisciplinary course in Safety in Surgery and Anaesthesia focused in training high performance teams with the collaboration of the Anaesthesia and Intensive Care National Incident Reporting System (SENSAR) and industry stakeholders. The workshop, intended for surgeons, anaesthesiologist, nurses and other non-surgical professionals, was held at a national level. The goal of our study was to compare the results of a survey administered before and after the workshop to the participants at HGUGM and the other Spanish hospitals.Material and Methods: The course design included 2 phases: a face-to-face seminar and online content delivery. During the seminar, a brief introduction about patient safety was made, followed by sessions on critical incident, underlying factors, human factor, CRM (crisis resource management), debriefing method and non-technical skills. Using video simulation, we recreated different scenarios in which patientu2019s safety has been compromised. Before and after the workshop a survey was provided to participants querying the following items: * Pre-session: general data of the participants, general knowledge about patient safety, human factor and teamwork, CRM, learning discussions and emotion, behaviour and communication skills.* Post-session: survey emphasised in: relevance of what they learnt for daily work, the impact in safety patient, communication skills, and the necessity for repeated training. Results:Nine sessions with a total of 159 participants were held in Spain from January to November of 2016. Six of them took place at our hospital. We included 90 participants in groups of 15 in each session (anaesthesiologist, surgeon and nurse of the same surgical field), as well as other non-surgical professionals (haematologist, neonatologist, pharmacists, etc.). Although most participants had prior training on safety, a lack of formation in non-technical skills figures prominently in our hospital (73%) and others (62%); as well as an absence of adequate coordination of the surgical team. Similarly, in all the hospitals, (77% HGUGM - 72.7% others), the participants had never received a workshop like this, neither know-how to implement a debriefing (61.4% and 50%) or CRM methodology. All the surveys show, when we compare our hospital to the rest of the Spanish hospitals, a p>0.05 therefore, we can conclude that this workshop can be transferred to all other hospitals, with similar results. Conclusions:Training workshops for multidisciplinary high performance teams is a tool capable of Improving Safety Culture in Surgical Environments. This workshop is a new tool to enhance no-technical skills and to improve coordination between professionals. Data suggest that periodic training is necessary to maintain the formation level.
