Paying the hospital analyzes in depth and compares the methods of paying hospitals across the principal developed countries - France, Germany, England, Holland, Switzerland, Canada, and the United States. It goes on to analyze the comparative effects of different payment methods - either within individual countries or across many countries - on administration, cost cointainment, and clinical performance. The volume is organized by topic, not country-by-country description, and offers information in the following sequence: methods of cost accounting, how budgets are screened and how rates are described, units of payment, how capital is distributed for buildings and equipment, deciding wages of hopital doctors, the organization of medical staffs, the work of financial managers, effects of different methods on levels of costs, volume, types of services, quality of care, and so on.

Mr. Burdett's labours in the cause of pay hospitals are too well known to need mention at our hands. To him is due the origin and subsequent development of the Home Hospitals Association, and he is generally recognised as the authority on the particular subject to which the present book relates. Mr. Burdett is extremely earnest in his advocacy of the system of payment-at any rate in part-for the benefits received in hospital, and he vigorously denounces the evils to which the present indiscriminate system of medical relief gives rise. He examines carefully, and with much painstaking detail, the working of the various systems of pay hospitals in vogue in France, Switzerland, Germany, Austria, Spain, Italy, Sweden and Norway, America, Canada, and the Colonies; and he expresses the opinion, in which we cordially concur, that middle-class or pay hospitals, if established in England on a proper basis, would probably become very popular and financially successful. In support of this view, he gives the opinions of gentlemen, who have given special attention to this particular question, and he also goes at great length into the reasons which make such hospitals desirable. No one, indeed, can rise from a perusal of Mr. Burdett's statements without feeling that the establishment of pay hospitals and pay wards would meet a real want in this country. Hospitals and hospital treatment are undoubtedly becoming more and more popular with the general public, and this result is hardly surprising. But it is none the less a grievous scandal that those who can afford to pay should be allowed fo go to these establishments to the exclusion of their poorer neighbors without in any way contributing to the funds of the hospital. Mr. Burdett proposes that whilst poor persons who can afford to contribute something towards their maintenance should be treated in the wards of the present general hospitals, the remunerative paying patients should be treated in separate hospitals of their own. In this way he would get over a difficulty which has been felt by many to be a great stumbling-block in the way of the extension of this desirable reform; and the arguments adduced in favor of this plan are certainly very cogent. Mr. Burdett gives an interesting account of a now forgotten institution, the first Pay Hospital established in England, by Dr. South wood Smith in 1840, together with a critical and exhaustive discussion of the efforts recently made by the treasurer of St. Thomas's Hospital to engraft the system of pay patients upon that institution. A lengthy chapter is devoted to the Home Hospital Movement, in which Mr. Burdett is more particularly interested. The importance of the objects which this useful society aims at fulfilling is very well brought out. We are gratified to find Mr. Burdett insisting strongly on the provision of hospitals for infectious diseases for the well-to-do, and of convalescent hotels, the need of both of which is now very grievously felt. The Home Hospitals Association, and the cause of pay hospitals generally, has a very earnest and powerful advocate in Mr. Burdett, whose book should be in the hands of every hospital manager in the country who desires to rescue the institution in which he is interested from the chronic impecuniosity to which but too many hospitals seem to be subject. -The Sanitary Record [1880]

No two markets for voluntary health insurance (VHI) are identical. All differ in some way because they are heavily shaped by the nature and performance of publicly financed health systems and by the contexts in which they have evolved. This volume contains short structured profiles of markets for VHI in 34 countries in Europe. These are drawn from European Union member states plus Armenia Iceland Georgia Norway the Russian Federation Switzerland and Ukraine. The book is aimed at policy-makers and researchers interested in knowing more about how VHI works in practice in a wide range of contexts. Each profile written by one or more local experts identifies gaps in publicly-financed health coverage describes the role VHI plays outlines the way in which the market for VHI operates summarises public policy towards VHI including major developments over time and highlights national debates and challenges. The book is part of a study on VHI in Europe prepared jointly by the European Observatory on Health Systems and Policies and the WHO Regional Office for Europe. A companion volume provides an analytical overview of VHI markets across the 34 countries.

Diagnosis Related Group (DRG) systems were introduced in Europe to increase the transparency of services provided by hospitals and to incentivise greater efficiency in the use of resources invested in acute hospitals. In many countries, these systems were also designed to contribute to improving – or at least protecting – the quality of care. After more than a decade of experience with using DRGs in Europe, this book considers whether the extensive use of DRGs has contributed towards achieving these objectives. Written by authors with extensive experience of these systems, this book is a product of the EuroDRG project and constitutes an important resource for health policy-makers and researchers from Europe and beyond. The book is intended to contribute to the emergence of a ‘common language’ that will facilitate communication between researchers and policy-makers interested in improving the functioning and resourcing of the acute hospital sector. The book includes: A clearly structured introduction to the main ‘building blocks’ of DRG systems An overview of key issues related to DRGs including their impact on efficiency, quality, unintended effects and technological innovation in health care 12 country chapters - Austria, England, Estonia, Finland, France, Germany, Ireland, the Netherlands, Poland, Portugal, Spain and Sweden Clearly structured and detailed information about the most important DRG system characteristics in each of these countries Useful insights for countries and regions in Europe and beyond interested in introducing, extending and/ or optimising DRG systems within the hospital sector

What are public health services? Countries across Europe understand what they are or what they should include differently. This study describes the experiences of nine countries detailing the ways they have opted to organize and finance public health services and train and employ their public health workforce. It covers England France Germany Italy the Netherlands Slovenia Sweden Poland and the Republic of Moldova and aims to give insights into current practice that will support decision-makers in their efforts to strengthen public health capacities and services. Each country chapter captures the historical background of public health services and the context in which they operate; sets out the main organizational structures; assesses the sources of public health financing and how it is allocated; explains the training and employment of the public health workforce; and analyses existing frameworks for quality and performance assessment. The study reveals a wide range of experience and variation across Europe and clearly illustrates two fundamentally different approaches to public health services: integration with curative health services (as in Slovenia or Sweden) or organization and provision through a separate parallel structure (Republic of Moldova). The case studies explore the context that explain this divergence and its implications. This study is the result of close collaboration between the European Observatory on Health Systems and Policies and the WHO Regional Office for Europe Division of Health Systems and Public Health. It accompanies two other Observatory publications Organization and financing of public health services in Europe and The role of public health organizations in addressing public health problems in Europe: the case of obesity alcohol and antimicrobial resistance (both forthcoming).

New York Times bestseller Business Book of the Year--Association of Business Journalists From the New York Times bestselling author comes an eye-opening, urgent look at America's broken health care system--and the people who are saving it--now with a new Afterword by the author. "A must-read for every American." --Steve Forbes, editor-in-chief, FORBES One in five Americans now has medical debt in collections and rising health care costs today threaten every small business in America. Dr. Makary, one of the nation's leading health care experts, travels across America and details why health care has become a bubble. Drawing from on-the-ground stories, his research, and his own experience, The Price We Pay paints a vivid picture of the business of medicine and its elusive money games in need of a serious shake-up. Dr. Makary shows how so much of health care spending goes to things that have nothing to do with health and what you can do about it. Dr. Makary challenges the medical establishment to remember medicine's noble heritage of caring for people when they are vulnerable. The Price We Pay offers a road map for everyday Americans and business leaders to get a better deal on their health care, and profiles the disruptors who are innovating medical care. The movement to restore medicine to its mission, Makary argues, is alive and well--a mission that can rebuild the public trust and save our country from the crushing cost of health care.
