Abstract:The purpose of this study was to obtain normative data to contribute to the limited body of knowledge of pediatric gait norms, and also to demonstrate the reliability of the simple, non-invasive gait analysis technique of footprint analysis. Eleven 5 year olds and seven 3 year olds were studied. All subjects had no known orthopedic, neurological, or developmental problems. The gait parameters studied were stride length, step length, and step width, and computations were performed to obtain velocity and cadence. Functional leg length was measured on the barefoot subjects. Three 35 foot long pieces of paper were taped to the floor for three trials. All first trials were discarded. Each child's feet were placed on an ink pad and the child then walked to the opposite end of the paper. The time was recorded for the child to walk ten steps within the middle 15 foot area of the paper. Means, standard deviations, and t-values were computed using the SPSSX data analysis program. This important monograph summarizes a comprehensive study on the maturation of walking in normal children. Research, undertaken at one of the world's leading gait analysis centers, involved over 400 studies on a total of nearly 300 children in ten age-groups from one to seven years. Data are presented on anthropometric measurements; tests of developmental progress; time/distance parameters such as stride length and walking velocity; twelve joint angles on each side measured throughout the gait cycle; dynamic electromyography of phasic activity in seven lower-extremity muscle groups; and force measurements including vertical force, fore/aft shear, medial/lateral shear and torque. At each age, composite joint-angle graphs and time/distance parameters are brought together with film tracings of a representative child in that age group. In addition, advanced methods of statistical analysis have been applied to the joint-angle data to define prediction regions within which ninety-five percent of normal children should lie throughout the gait cycle. Finally, a "decision tree" is presented from which a fitted age can be inferred for a subject based on non-age-specific data gathered in a motion analysis lab. Practical applications are demonstrated in a chapter devoted to two case studies.

The Glenrose Hospital, Edmonton is considering routine clinical use of computerized gait analysis techniques in the management of children with cerebral palsy or spina bifida who have significant walking disorders. About 100 children per year could be examined to assist with decisions on their treatment. Evidence of clinical benefit from use of modern gait analysis techniques is sparse. These technologies seem helpful in detecting within-subject gait changes, and between-subject gait differences. However, available evidence is insufficient to draw conclusions about the effects of computerized gait analysis on treatment outcomes. Cost per examination with a facility of the sort proposed by the hospital might be of order of $1,800 - $2,200. Cost per patient would depend on the extent of follow-up gait analysis after treatment. On the basis of information available for this report, computerized gait analysis is a potentially useful technology in the clinical management of persons with walking disabilities, but its efficacy is not established. It is suggested that, if computerized gait analysys is adopted for routine clinical use by the Glenrose hospital, it should be regarded as a developing technology and its application linked to systematic collection and assessment of patient outcomes and cost data under well-defined protocols.
