A who perinatal study group was set up in 1979 to study and report on the issues surrounding birth care in Europe and this is their final report. They describe and review the official services, the alternative services, and the information and evaluation services. Information gleaned ranged from systems of birth and death registrations, patterns of antenatal visits and provision of intensive care services, to who is allowed to be present during a birth, and what tests and procedures are administered routinely. The variation in the rates for many procedures from country to country, without any resultant variation in mortality or morbidity rates, shows that the use of many of them could be usefully reviewed. Based on these findings, the final chapter summarizes the present state of services for women and their babies during pregnancy and birth and following birth, and analyses present trends and future needs.

The study examined medical and social services to pregnant women, newborns and their families in 10 European countries and suggested implications for policy and practice in the United States. The 10 countries (Belgium, Denmark, France, Federal Republic of Germany, Ireland, Netherlands, Norway, Spain, Switzerland, and the United Kingdom) tend toward better birth weights and infant survival than does the United States. Important demographic differences between the United States and Western Europe are highlighted and include substantially lower rates of teenage pregnancy, abortion and childbearing in Europe. Key reasons for better European performance are identified as: (1) easily understood and available provider systems; (2) removal of all barriers (especially economic) to services; and (3) linking of prenatal care to comprehensive social and financial benefits. Financing systems for health care varied among countries with insurance and social security programs predominant. Individual chapters look at study purpose, method and sources, national characteristics, maternity related services and benefits, considerations for U.S. policy, conclusion and national synopses. It is concluded that the issue of infant survival is neither medical nor financial but political. (DB)

Perinatal Medicine is a relatively new specialty, sited between the mechanistic approach of traditional obstetrics and the anticipatory and preventative out look expressed in the study of fetal growth which extends into monitoring neo natal progress and development. It is of primary importance that obstetricians and neonatologists should think alike and should not allow their interests to develop along separate lines. Frequent clinical consultations with neonat ologists cooperating in prenatal care and obstetricians visiting the special nur sery on a regular basis are essential if every fetus is to reach its full potential. Such aims have been advanced by previous European Congresses and it was our privilege in Dublin to host the IXth Congress held at the Royal Dublin Society from September 3rd to 5th 1984. Over nine hundred delegates rep resenting thirty-nine countries attended the scientific and social programme. The theme of the congress was the mature baby. The organizers felt that while major advances had been made in the management of prematurity there were still far too many mature fetuses dying and too many deaths in the nor mally formed appropriate weight for gestational age neonate. Hence antenatal fetal assessment, the management of labour including electronic monitoring in normal women and asphyxial brain damage were major topics. Pregnancy hy pertension, caesarean section, breech presentation, diabetes and unexplained mature stillbirth were workshop topics.
