Around the world, the population of geriatrics is increasing. Health related problems of geriatrics require special attention because these people face many problems, which includes problems associated to health, social support and economic uncertainty. A drug-related problem (DRPs) is an episode or situation involving drug therapy that essentially or theoretically interferes with patient's anticipated health outcomes. DRP is the third or fourth leading cause of death in the elderly. The aim of the study was to evaluate effectiveness of pharmaceutical care programme on health outcome of geriatric patients. The present study was a prospective interventional study carried over a period of three year and the study was performed in 3 phases such as pre-intervention, intervention and post intervention. The drug therapy details of the patients were collected from pre-rounds, ward rounds with physician and inpatient case records. Pharmacist reviewed the drug therapy, identified the drug related problems during ward rounds and discussed with physician and suitable suggestions was provided which had been documented. MMAS-8 and SF-36 are used in the study to find out medication adherence and quality of life respectively. During the study period 770 patients data was collected, out of which 58% were males and 42% were females. The maximum patients were in the age group of 60-70 range (50.4%).A total of 813 drug related problems were identified from 770 study subjects. The number of drug related problems was predominant in males than females. The most common drug related problems was found to be drug interactions (40.9%). Patient counseling and health care information leaflet are the most interventions in the study. Providing patient counseling, it yields a positive impact on patient compliance and their quality of life. The acceptance rate of suggestions and the changes in drug therapy was found to be high (40.1%). Descriptive analysis, chi-square test and t-test are used in this study for statistical analysis. Pharmacist involvement in inpatients care can significantly help to identify, resolve and prevent the drug related problems. Outcomes of interventions indicate that almost 80.26% of problems are totally solved by pharmacist's interventions in this study. Pharmacist in geriatric health care has a positive influence on their health outcomes.

Home health care, as an organized entity in the continuum of health care in the U.S., is a fairly new phenomenon. Geriopharmacotherapy in Home Health Care looks at the state of the pharmaceutical arena in home health care. With origins in a federally funded grant program, five state programs--North Carolina, Hawaii, Mississippi, South Carolina, and Utah--have evolved into a progressively interactive group, with certain common problems and perceptions across states, both in medications and other programmatic concerns. The contributing authors of this book examine several of these problems and concerns in detail. Each chapter assists readers in understanding the pharmaceutical endeavors undertaken through the initiative and creativity of these state-funded home health programs, which are still developing and growing. There are multiple concerns about the impact of medication on patients cared for by home health care workers, among them, the high incidence of chronic diseases, debilitating illnesses, and the potential for adverse reactions to medication. Several of the topics explored in detail include: a critical analysis of the medications patients consume how to clinically critique problems and seek interventions to remedy therapeutic and clinical problems survey results on pharmaceutical services from home health agencies in North Carolina needed modifications in available computer software to provide optimal computer capabilities for consultant pharmacy services an examination and comparison of several software programs for practicality and logistical capabilities Geriopharmacotherapy in Home Health Care documents the outgrowth of program innovation and the creativity of five states’current status of home health care pharmacy. With continued tremendous growth in the elderly population, the demands on home health care will continue to grow dramatically for years to come. Home health care professionals--nurses, nurses aids, consultant pharmacists, agency directors and workers--will find the developments in this book informative as they increase and improve the services available to homebound elderly.

Learn how to talk—and listen—to your elderly clients Pharmacy Practice in an Aging Society examines the aspects of aging that transcend physical and biological changes, addressing practical concerns such as communication, understanding values, and end-of-life issues. This unique book provides pharmacists with the basics on how to enhance their service to the elderly and offers additional suggestions for improving the empathy and understanding they have for their patients. The book explores general health and pharmacy concerns of the elderly, including abuse, neglect, sexuality, stress, and addictions, and presents methods of helping seniors stay healthy through exercise and with appropriate interventions. 34 figures, tables, and exhibits make complex information easy to access and understand. Fewer than five percent of persons over 65 end up in a nursing home, and many stay only long enough to recover or rehabilitate before returning home. The other 95 percent need our attention and our care. Pharmacy Practice in an Aging Society helps pharmacists develop the special tools they need to deal with the elderly, identifying the most challenging areas, and providing effective frameworks for addressing them. The book also includes sidebars that summarize important ideas, including: “Communication”—choosing the right words when delivering difficult or even unwelcome information to patients “Ethics on the Spot”—anticipating difficult questions and determining if information is appropriate to pass along to a patient “Statistics at a Glance”—comparative statistics presented in a way that makes a point “Cutting Edge”—current research findings and what they mean to your patients “Too Late Now”—when changing a patient’s behavior or adding a medication is unlikely to help “Then and Now”—compares past treatment and approaches with current ones and much more! Pharmacy Practice in an Aging Society is an essential resource for community and hospital pharmacists, educators, and anyone else with an interest in health.

Medication mishaps can and do happen in nursing homes. This highly important new book focuses on the scope of drug-related problems (DRP) of misuse and adverse reaction to medications in the nursing home environment. Experts provide practical methods for detecting DRPs, they explain a systematic approach to preventing DRPs, and they document the magnitude of DRPs in the long term care setting. Each chapter of this timely book deals with a different type of drug-related problem and includes operational definitions, prevalence of the problem, and the actions needed to detect and minimize risk and prevent recurrence. A checklist at the end of each chapter aids readers in assessing specific problems in nursing homes, home health care, day care settings, and other locations in which medications are given to the elderly. Illustrative case examples are used throughout the text. With an emphasis on awareness and change, Drug-Related Problems in Geriatric Nursing Home Patients will be invaluable for pharmacists, physicians, and nurses who work in long term care; health care planners who need to be cognizant of the scope and magnitude of medication mishaps in the nursing home; administrators who are ultimately responsible for such problems. Students and educators will find the information, and especially the checklists, timely and practical.

This work is intended as a brief but focused compilation to assist with diagnosis and management of the most common serious medical problems in the rapidly growing geriatric population. The geriatric population recently expanded by the fact that the baby boomers have reached the milestone of 65 years of age in the past 5 years. Tips for diagnosis, medication administration, and logistics of cost-effective management in the health-care continuum are presented in this book. The latter often consists of a journey from home to medical office to emergency room to hospital bed to intensive care unit to long-term acute care hospital to skilled nursing facility to long-term residential facility and/or back home, which is also reviewed in the book Geriatrics.
