En 1991, la France - stupéfaite - découvre l'ampleur de la contamination par le virus du sida au cours de transfusions sanguines. C'est un véritable séisme qui, en ébranlant les certitudes, met au jour la fragilité d'un système, où les comportements humains sont tout autant déterminants que la science. Cette révélation soulève bien des questions, quant aux décisions prises depuis l'apparition du sida en 1981, interrogations qui s'étendent à l'ensemble de la politique de dépistage, pilier contesté de la lutte contre le sida à partir de 1985, car le seul outil scientifique disponible, le test VIH, n'est pas un test de dépistage comme les autres. Son utilisation représente un véritable enjeu de société et de santé publique, par les multiples questions qu'il soulève et les conséquences qu'il implique. Face à la singularité du sida, le choix d'un dépistage obligatoire, systématique ou volontaire, se présente comme une question problématique, qui déborde le cadre technique et professionnel, pour devenir un problème politique. Ce mélange inséparable de science et de politique, aboutit à des choix qui ne deviennent intelligibles qu'à la lumière d'une analyse des jeux et des relations de pouvoirs, qui s'organisent entre les différents acteurs impliqués : administrations responsables, politiques, médecins, associations et usagers. Dans cette perspective, une approche comparative des processus de décision - dans trois pays européens - au cours de la première décennie sida, s'avère éclairante ; elle permet, face à la confusion et à l'hétérogénéité des pratiques nationales, de faire émerger une réponse à la double question : comment expliquer les différents choix, et à quelles conditions les politiques de dépistage contribuent-elles vraiment à la lutte contre la propagation de l'épidémie ?

La construction de l'action publique contre le sida en Afrique est un processus dynamique et complexe qui met en mouvement une pluralité d'acteurs intervenant au niveau local, mais aussi des acteurs extérieurs appartenant à la sphère internationale. Au Burkina Faso, pays sous régime d'aide, elle a donné lieu à des formes de mobilisations qui ont vu la participation active de la "société civile", mue par la volonté de s'approprier le problème-sida dans un élan solidaire avec les personnes infectées et en synergie avec la puissance publique. Pour être intelligibles, ces dynamiques locales doivent cependant être décryptées à la lumière de l'engagement et de l'influence des partenaires extérieurs, véritables penseurs et financeurs des programmes de santé réappropriés par les acteurs au niveau local. Les interventions opérées sur le terrain par les associations et leurs leaders, pour réduire l'impact sanitaire et social du sida s'intègrent, peu ou prou, dans le droit fil des prescriptions dictées par ces décideurs internationaux. Ce travail propose une analyse focalisée sur les recompositions successives de l'action publique au Burkina Faso, en mettant en exergue le rôle et la participation des ONG/associations, en tant que partenaires et adjuvants de l'Etat. Au-delà des enjeux majeurs des politiques publiques définies par l'autorité centrale, il appréhende les formes d'action construites par les acteurs associatifs, analyse les postures, les stratégies, les ressources et les marges d'action dont dispose le mouvement associatif dans son engagement contre le sida, mais aussi dans ses rapports avec les pouvoirs publics et les partenaires au développement. La réflexion se nourrit de l'apport de la socioanthropologie et de la sociologie politique et tente de saisir de manière diachronique les dynamiques (sociales, politiques, économiques) qui se jouent dans le champ de la lutte contre le sida au Burkina Faso depuis près de trois décennies.

In the mid-1980s public health officials in North America, Europe, Japan, and Australia discovered that almost half of the hemophiliac population, as well as tens of thousands of blood transfusion recipients, had been infected with HIV-tainted blood. This book provides a comparative perspective on the political, legal, and social struggles that emerged in response to the HIV contamination of the industrialized worlds blood supply. It describes how eight nations responded to the first signs that AIDS might be transmitted through blood, and how they falteringly arrived at and finally implemented measures to secure the blood supply. The authors detail the remarkable saga of the mobilization of hemophiliacs who challenged the state, the medical establishment, and even their own caregivers as they sought recompense and justice. In the end, the blood establishments in almost every advanced industrial nation were shaken. In Canada, the Red Cross was forced to withdraw from blood collection and distribution. In Japan, pharmaceutical firms that manufactured clotting factor agreed to massive compensation -- $500,000 per hemophiliac infected. In France, blood officials went to prison. Even in Denmark, where the number of infected hemophiliacs was relatively small, the struggle and litigation surrounding blood has resulted in the most protracted legal and administrative conflict in modern Danish history. Blood Feuds brings together chapters on the experiences of the United States, Japan, France, Canada, Germany, Denmark, Italy, and Australia with four comparative essays that shed light on the cultural, institutional, and economic dimensions of the HIV/blood disaster.
