Highly pathogenic avian influenza (HPAI) represents a threat to poultry industries worldwide and to people's livelihoods, and a potential threat to human health. The international community has a vested interest in minimizing the spread of this disease. Countries may be under threat of introduction of HPAI through unregulated poultry trade and marketing practices and, on rare occasions, exposure of poultry to wild birds, especially waterfowl. FAO and the World Organisation for Animal Health have prepared the first edition of this manual to help national animal health authorities and other stakeholders prepare for a possible incursion of HPAI, detect disease as soon as possible and respond as rapidly as possible to contain the disease. The Ministry of Health, Malaysia (MOH) and the Department of Veterinary Services, Malaysia (DVS) have been holding annual field simulation exercises since 2006, focusing on outbreaks of Highly Pathogenic Avian Influenza (HPAI) involving multiple stakeholders. This exercise is designed to facilitate understanding as well as to review the applicability of SOPs and relevant guidelines in the management of HPAI outbreak. It further aims to identify the strengths and opportunities for improvement in all aspects of outbreak management. This module is an example of possible scenarios of the exercise that depicts a hypothetical outbreak of avian influenza amongst poultry, either commercial or backyard. While there are several specific objectives, the general objective is to enhance and strengthen interagency preparedness and response to HPAI outbreak at all levels.

A highly pathogenic strain of avian influenza (AI) has spread to nearly 60 countries over the past few years, killing millions of birds and more than 170 humans. Controlling the virus in poultry is key to reducing the risk of a human pandemic. The Department of Agriculture (USDA) is responsible for planning for AI outbreaks in poultry, with states' assistance. The Department of Homeland Security (DHS) is responsible for coordinating the federal response for certain emergencies and developing policy documents that serve as a basis for national emergency planning. GAO described the steps USDA is taking to prepare for highly pathogenic AI and identified key challenges. GAO reviewed response plans, statutes, and regulations; visited poultry operations; interviewed federal, state, and industry officials in five states that experienced outbreaks; and reviewed 19 state plans.

Avian influenza, or 'bird flu', is a contagious disease of animals caused by viruses that normally infect only birds and, less commonly, pigs. Avian influenza viruses are highly species-specific, but have, on rare occasions, crossed the species barrier to infect humans. In domestic poultry, infection with avian influenza viruses causes two main forms of disease, distinguished by low and high extremes of virulence. The so-called "low pathogenic" form commonly causes only mild symptoms (ruffled feathers, a drop in egg production) and may easily go undetected. The highly pathogenic form is far more dramatic. It spreads very rapidly through poultry flocks, causes disease affecting multiple internal organs, and has a mortality that can approach 100%, often within 48 hours. A pandemic can start when three conditions have been met: a new influenza virus subtype emerges; it infects humans, causing serious illness; and it spreads easily and sustainably among humans. The H5N1 virus amply meets the first two conditions: it is a new virus for humans (H5N1 viruses have never circulated widely among people), and it has infected more than 100 humans, killing over half of them. No one will have immunity should an H5N1-like pandemic virus emerge. All prerequisites for the start of a pandemic have therefore been met save one: the establishment of efficient and sustained human-to-human transmission of the virus. The risk that the H5N1 virus will acquire this ability will persist as long as opportunities for human infections occur. These opportunities, in turn, will persist as long as the virus continues to circulate in birds, and this situation could endure for some years to come.
