Recurrence is a significant public health problem. So, this fourth Suvretta meeting, held in February 2006 is intended to concentrate on this problem. Technical nuances of the various operations have been discussed to pursue consensus concerning the best techniques. Methods were explored to improve surgeons' education and look into the multifactorial etiologies to understand the biology of hernia recurrence better.

Even the best hernia repair can result in postoperative difficulties for the patient caused by repair sequelae as for example pain, infertility, infection, adhesion and dislocation of the protheses. That can happen many years later and now, where the general principle of hernia repair is well understood all over the world, these sequelae are noticed more and more. To define them, to evaluate the absolute and relative risk of these sequelaes and to describe the ways of their prevention, diagnosis and treatment, the 5th Suvretta meeting had focussed on this subject. We discussed if there’s a principle risk by technique, material or both. The results of these discussions and the future handling and evaluation of this problem was the aim of this meeting. Even the best method can be made better by optimization of its single components. Even the best hernia repair can result in postoperative difficulties for the patient caused by repair sequelae such as pain, infertility, infection, adhesion and dislocation of the prostheses. The 5th Suvretta meeting was held in order to define these sequelae, to evaluate the absolute and relative risks they pose, and to discuss the methods of their prevention, diagnosis and treatment. Laparoscopic Inguinal Hernia Repair written by Ara Darzi and has been published by CRC Press this book supported file pdf, txt, epub, kindle and other format this book has been release on 1994 with Medical categories.

The repair and management of inguinal hernias represents a significant part of the general surgeon's workload. It was therefore inevitable that following the success of laparoscopic cholysysectomy, surgeons would develop a procedure for repairing inguinal hernias laparoscopically. This book provides the first comprehensive account of laparoscopic inguinal hernia repair. The Editors' aim has been to give step-by-step guidance to each operative procedure discussed with reference to long term results. One of the biggest problems facing surgeons learning new laparoscopic procedures is an understanding and orientation of basic anatomy when viewed through the laparoscope. This issue is discussed at length with guidance on how to avoid the common pitfalls. Final chapters look at the advantages and disadvantages of the Lichtenstein open mesh hernia repair approach, safety and properties of non-absorbal mesh with the last chapters concentrating on laparoscopic suture repair and the new mini hernia operation.

This practical, go-to reference by Drs. Losken and Janis will provide you with the tools you need when addressing a host of abdominal wall problems, ranging from simple to complex hernia repair and abdominal wall reconstruction. It provides information on the latest developments in mesh and biologic materials and their applications for abdominal hernia repair as well as step-by-step surgical technique for ensuring successful outcomes in all types of abdominal wall reconstruction. Advances in Abdominal Wall Reconstruction presents the range of current options available for surgical repair in a systematic fashion. Treatment options and algorithms provide readers with insights into choices for approaching different problems. Strategies for minimizing morbidity and for avoiding recurrences are also discussed in detail as are helpful tips and tricks and safe and reliable options for dealing with complex problems. New techniques are described along with the evidence-based data on outcomes and numerous case examples to reinforce important concepts. Potential problems and pitfalls are thoroughly covered to help surgeons avoid potential complications. Although the main focus of this book is on surgical technique, it also discusses basic principles, etiology of abdominal defects, organizations strategies and outcomes. A DVD with 3 operative videos complements the book. The editors have put together an imminently helpful work which should prove invaluable for a broad range of surgical specialties. They are assisted by a multi-disciplinary group of leading experts and pioneers in surgery and plastic surgery. The book is divided into three parts. The first part focuses on fundamentals with essential chapters devoted to abdominal wall anatomy, hernia defect classification, abdominal wound care and synthetic meshes, and tissue biologics for abdominal wall reconstruction. Parts II and III are devoted to various types of abdominal wall reconstruction as well as simple and complex hernia repair using synthetic meshes and biologics. Chapters within these sections are clinical and technique-oriented, with ample illustrations and photos depicting the different surgical approaches. A key chapter in this section focuses on "Anatomic Components Separation: Applications and Advances in Technique." This simple, reliable technique is one of the most exciting, recent advances in abdominal wall reconstruction. Another hot topic covered in depth in several comprehensive chapters is that of biologics in complex abdominal wall repair; these materials have revolutionized abdominal wall reconstruction. Critical chapters are included on the science behind biologics as well as specific techniques for using them. Other cutting-edge topics include: laparoscopic hernia repair and management of parastomal hernias, enterocutaneous fistulas, postoperative abdominal bulges (a common, challenging problem), and recurrent ventral abdominal hernias, to name a few. An exciting chapter on abdominal wall transplantation gives readers insight into promising new developments and what lies on the horizon in abdominal wall reconstruction. This textbook provides a comprehensive, state-of-the art review of the field of hernia surgery, and will serve as a valuable resource for clinicians, surgeons and researchers with an interest in both inguinal and ventral/incisional hernia. This book provides an overview of the current understanding of the biologic basis of hernia formation as well as laying the foundation for the importance of hernia research and evaluating outcomes in hernia repair. Diagnosis and management strategies for inguinal and ventral hernia are discussed in detail with separate techniques sections for the most widely used procedures in this field as well as emerging technologies such a robotic and single incision surgery. Pertinent associated topics to inguinal hernia surgery such as chronic groin and athletic pubalgia are covered in detail. For incisional hernias, associated topics such as hernia prevention and enhanced recovery protocols are discussed. For both inguinal and ventral/incisional hernias mesh choices and available mesh technologies are discussed in detail as this remains an often confusing matter for the general surgery. When appropriate, chapters to highlight controversies in care are featured such as the use of synthetic mesh in contaminated surgery and laparoscopic closure of defects in laparoscopic ventral hernia repair. Current recommendations and outcomes data are highlighted when available for each technique. Textbook of Surgery will serve as a very useful resource for physicians and researchers dealing with, and interested in, abdominal wall hernias. It will provide a concise yet comprehensive summary of the current status of the field that will help guide patient management and stimulate investigative efforts.

Primary and incisional ventral hernias are common conditions often encountered in surgical practice. Because of the frequency of this problem it has come to be managed by surgeons in general, regardless of the type of hospital or the conditions dealt with in their daily practice. Laparoscopic surgery has demonstrated to have an important role among the different technique described to repair ventral hernia with less recurrent rate, less morbidity and less overall cost than open conventional repair, with all the advange of the laparoscopic approach. As a result the indications for this surgical technique are currently being debated since the advantages are evident and progressive implementation is ensured. Now is the time to analyze the usefulness, results, technical variants, anatomic, physiologic and scientific basis and implications involved in implementation of laparoscopy as the technique of choice.
