In recent years, a considerable number of countries have developed and implemented strategies aiming at reducing inequalities in health. However, knowledge of effective policies or strategies to reduce socioeconomic inequalities in health is still very fragmented. The aim of this book is to describe and compare different European health equity strategies and their potential successes. The main part of the material comes from a comparative study of national public health strategies for equity in health with the following participating countries: Denmark, Finland, England, Italy, Netherlands, Norway, Spain and Sweden. National experts wrote the country chapters which not only cover questions concerning whether their countries were taking measures on the individual public health problems U+2013 such as smoking, alcohol or physical activity U+2013 but also if the policies had considered the wider, social determinants of health and experiences of the potential implementation processes. This publication presents an up-to-date picture concerning equity-oriented public health policies in Europe. However, there is considerable lack of relevant data to enable comprehensive comparisons and analyses of successes and problems with the implementation and monitoring of health equity policies. We hope the book will stimulate continued efforts to harmonise relevant data collections and improve collaborative analyses aiming at shaping an evidence base for policy decisions about the most effective ways to promote equity in health.

Investing for health and well-being is a driver and an enabler of sustainable development and vice versa and it empowers people to achieve the highest attainable standard of health for all.

Countries in Europe have long recognized that good health care, though essential, is not in itself sufficient to improve health or to reduce the increasing gaps in health status between the rich and the poor. In 1984, together with WHO, they adopted and many attempted actively to implement, what has become known as the health for all policy. This called for a radical shift from health services planning to an approach based on setting objectives and targets for health, requiring partnerships with industry, agriculture and commerce and settings such as workplaces and schools. It also required changes in behaviour and action to ensure a fairer distribution of the determinants of health, such as income, education, employment opportunities, and adequate food and housing. This volume provides a broad overview of how the 51 countries in the WHO European Region set about this ambitious task. The study's findings are based on replies to checklists sent to the Member States, a set of country profiles (given as an annex), published policy documents, and case studies written by people who were involved in the process in their own countries or regions. Examining how countries used some of the policy instruments open to them as they strove to close the health gaps, the study explores how far the rhetoric was accompanied by the necessary action and whether countries were able to move from their traditional ways of working, and poses questions as to whether this approach will be sufficient to meet the challenges of the future.

There is a broad consensus across European states and the EU that social and economic inequality is a problem that needs to be addressed. Yet inequality policy is notoriously complex and contested. This book approaches the issue from two linked perspectives. First, a focus on functional requirements highlights what policymakers think they need to deliver policy successfully, and the gap between their requirements and reality. We identify this gap in relation to the theory and practice of policy learning, and to multiple sectors, to show how it manifests in health, education, and gender equity policies. Second, a focus on territorial politics highlights how the problem is interpreted at different scales, subject to competing demands to take responsibility. This contestation and spread of responsibilities contributes to different policy approaches across spatial scales. We conclude that governments promote many separate equity initiatives, across territories and sectors, without knowing if they are complementary or contradictory. This outcome could reflect the fact that ambiguous policy problems and complex policymaking processes are beyond the full knowledge or control of governments. It could also be part of a strategy to make a rhetorically radical case while knowing that they will translate into safer policies. It allows them to replace debates on values, regarding whose definition of equity matters and which inequalities to tolerate, with more technical discussions of policy processes. Governments may be offering new perspectives on spatial justice or new ways to reduce political attention to inequalities.
