Although prostaglandin El (PGE ) has been clinically available for a long 1 time, only in recent years has its effectiveness in peripheral arterial occlusive disease been confirmed in controlled studies. Not surprisingly, the favour able results achieved both in patients with critical limb ischaemia and in those with intermittent claudication has stimulated research activities into the clinical pharmacology of this prostaglandin. As a consequence of these efforts, exciting new findings have revealed that PGE has anti thrombotic , endothelium-stabilizing and leucocyte-stabi 1 lizing properties as well as effects on lipid metabolism, all of which, quite apart from its well-known anti-aggregating and vasodilator effects, may add to the clinical efficacy of the substance. New data have also been gathered on the metabolism of PGE most b notably the detection of 13,14-dihydro-PGE a metabolite which was b recently isolated in humans following the administration of PGE . Being 1 biologically active, the pharmacodynamic spectrum of 13,14-dihydro-PGE 1 very closely resembles that of PGE . This finding may help to explain the 1 efficacy of PGE despite its rapid metabolization when given intravenously.

This is the second of a number of international symposia which will, I hope, continue to be held until atherosclerosis is no longer a major problem. The first symposium was held three years ago in Athens, Greece, under the chairmanship 1 of Dr. Constantinos]. Miras , who, although he could not attend this symposium, participated actively in the deliberations of this Program Committee. Atherosclerosis together with its sequelae constitute the most important source of morbidity and mortality in civilized countries. While a major attack is being made on the consequences, the sequelae of this disease, not enough attention is being paid to the basic cause, atherosclerosis. Yet, if the basic disease were eradicated, the major concern of this symposium, the sequelae would constitute only a minor and rare disease group. The approach of the Program Committee was to bring together experts in the multiple disciplines which have a bearing on atherosclerosis. There is a great need for an exchange of ideas from various groups studying the basic process in many divergent ways. The hope we have is that those present (or those later studying the Proceedings) may be stimulated to attack the problem in new ways. Perhaps a breakthrough will be made or, at least, a brick or two added to build the structure, a rampart needed to defend against atherosclerosis. Better still, their contributions may help to confine it to a small area.

Chronic critical limb ischemia remains one of the most frequent causes of illness and hospitalization in the western world. Interest in critical limb ischemia has increased markedly in recent years. There have been significant achievements in diagnostic pro cedures as well as in the medical, invasive, and surgical treatment of patients with critical limb ischemia. Despite a considerable research effort, the pathophysiological mechanisms of critical limb ischemia in humans remains to be established. Prominent is the increasing interest in the microcirculation, as well as in the hemorheological, hemostatic and inflammatory changes which may promote critical limb ischemia. Further research is necessary to quantify the relevance of microcirculatory in vestigations in terms of predicting the prognosis or to evaluate the effects of new therapies. Our main task is to offer the patient the best possible therapy in order to avoid limb loss and to improve quality of life. The various methods of treatment, their suc cess rates, and their complications, must be carefully evaluated and discussed. This volume presents the complex field of diagnosis and treatment of critical limb ischemia. The diagnosis and treatment involves different specialists including angio logists, interventional radiologists, vascular surgeons, plastic surgeons, diabeto logists, etc. Individual chapters have been written by Belgian and German experts in the various specialities, who present their own experience. We are very grateful that these experts have joined us to produce this book on the epidemiology, pathophysiology, and rational therapy.
