Nowhere is the injustice of the global distribution of income and wealth more palpable than in health. While the world’s affluent spend fortunes on the most trifling treatments, poor people’s lives are ruined and often cut short prematurely by challenges that could easily be overcome at low cost: childbirth, diarrhoea, malnutrition, malaria, HIV/AIDS, measles, pneumonia. Millions are avoidably dying from such causes each year and billions of lives avoidably blighted by these diseases of poverty. Drawing on in-depth empirical research spanning Asia, Latin America, and Africa, this path-breaking collection offers fresh perspectives from critically engaged scholars. Protecting the Health of the Poor presents a call and a vision for unified efforts across geographies, levels and sectors to make the right to health truly universal.

Abstract: Health systems are not just about improving health: good ones also ensure that people are protected from the financial consequences of receiving medical care. Anecdotal evidence suggests health systems often perform badly in this respect, apparently with devastating consequences for households, especially poor ones and near-poor ones. Two principal methods have been used to measure financial protection in health. Both relate a household's out-of-pocket spending to a threshold defined in terms of living standards in the absence of the spending: the first defines spending as catastrophic if it exceeds a certain percentage of the living standards measure; the second defines spending as impoverishing if it makes the difference between a household being above and below the poverty line. The paper provides an overview of the methods and issues arising in each case, and presents empirical work in the area of financial protection in health, including the impacts of government policy. The paper also reviews a recent critique of the methods used to measure financial protection.

One of the most urgent development challenges facing many low and middle income countries is the need for adequate financing systems to pay for health care provision to the estimated 1.3 billion impoverished people living in rural areas or working in the informal sector in urban areas. This publication considers ways of improving the financing of health care at low income levels, as part of a global strategy for increased investment in health and poverty reduction. Topics discussed include: global and regional trends in healthcare financing; strengths and weaknesses of community-based health financing, and experiences in Asia and Africa; country case studies using household survey analysis from Senegal, Rwanda, India and Thailand; deficit financing; and the impact of risk sharing on achieving health system goals.
