Interrogando a possibilidade da transmissão da Psicanálise no campo psiquiátrico e da saúde mental, tal como o vemos hoje, dediquei-me especialmente a examinar o clássico dispositivo psiquiátrico da Apresentação de Pacientes. Definido pela entrevista com o paciente como dispositivo clínico e de ensino, caracteriza-se pela presença do paciente, do entrevistador e do público. Sua renovação, realizada por Jacques Lacan, o transformou e relançou como um instrumento de trabalho pautado na ética psicanalítica. Mais do que nunca, parece necessário zelar e cuidar do que faz memória, do que faz história, e para tal, sublinho o percurso histórico da Psicanálise no Hospital Psiquiátrico de Jurujuba, na cidade de Niterói-RJ. Essa escolha se justifica pelo que lá se construiu, desde sua fundação até os dias de hoje, como um lugar de endereçamento, seja pelos profissionais e estudantes de psiquiatria e saúde mental, com tantos pedidos de formação, ou pelos pacientes, na demanda de acolhimento e tratamento. Diante da hegemônica prática medicamentosa nas intervenções terapêuticas aos sintomas e ao sofrimento dos pacientes, mesmo no campo da saúde mental, observamos uma tendência à drástica redução de uma clínica apoiada na palavra em transferência, colocando em risco a possibilidade mesma de que eles possam exercer sua cidadania dentro de um laço necessário para sua sustentação. Daí a relevância de investigar as condições, os dispositivos de possibilidade para uma (re)colocação da clínica em causa suportada pela transferência. De que se trata uma clínica em que se considere o sujeito? Essa questão não é nova, e encontra no livro uma possibilidade de examinarmos uma aposta, por meio do dispositivo das Apresentações Clínica de Pacientes. Um dispositivo que convida pacientes e clínicos a um trabalho subjetivo que permita um terreno mais sólido para as ações necessárias às decisões clínicas, em sua dimensão individual e social.

This book is recommended to psychoanalysts and therapists interested in the analytical technique, and particularly that work with patients who have deficits in their symbolization capacity. It presents studies of technical aspects of the analytical process with patients who are difficult to reach. Collusions named 'chronic enactments' show that the analytic dyad cannot dream and the analytical field is paralyzed without the analyst perceiving it. Chronic enactments are undone through unconscious acts or behaviours that threaten to destroy the analytical process: behaviours that are named 'acute enactments'. The thorough study of these enactments show that they take the dyad to an awareness of the discrimination between self and object and re-establish the capacity to dream. It is demonstrated that this occurs in an attenuated traumatic form, revealing in the analytical field the externalization of primitive non-dreamed traumas. Clinical, artistic, and mythical models are part of the discussion. The emphasis on clinical aspects allows readers to use different theories to consider the clinical facts. The clinical theories used by the author are mostly post-Kleinian and Bionian.
