L’alexithymie est un « style cognitif » qui se caractérise par une incapacité à verbaliser les émotions, une vie imaginaire peu développée, une tendance à recourir à l’action pour éviter ou résoudre les conflits, et un discours caractérisé par la description détaillée des faits, des événements, des symptômes physiques. L’alexithymie se retrouve préférentiellement dans la pathologie psychosomatique dont elle pourrait d’ailleurs être un facteur étiologique. Mais sa présence a été encore retrouvée dans certaines addictions (toxicomanie, alcoolisme) ou troubles mentaux (état de stress post-traumatique, dépression), ainsi que dans la population normale. Ce livre fait le point sur les travaux neuropsychologiques qui ont montré que l’alexithymie pouvait avoir plusieurs origines (absence de transmission de l’information entre les hémisphères cérébraux, réaction au stress, modification du traitement cognitif de l’émotion en mémoire à long terme...). Mais il s’intéresse aussi aux théories psychanalytiques qui, pour leur part, ont mis l’accent sur la différence entre les mécanismes à l’œuvre dans la névrose et ceux opérant dans l’alexithymie (refoulement de l’imaginaire pour Sami Ali, forclusion de l’affect pour J. McDougall...). La question des thérapeutiques de l’alexithymie est abordée tant à partir des positions cliniques actuelles que des prolongements de certaines formes de thérapies cognitivo-comportementales.

An old woman walks slowly up the hill from the store to her house. The hill is quite steep and the packages she carries, heavy. A month ago, as B. lay playing quietly in his crib, a toy key slipped out of his hand onto the floor. Almost immediately he turned his attention to another toy, close by, which he took up and put into his mouth. Yesterday, very nearly the same thing happened, except this time as soon as the toy key fell, he began to cry loudly, forcing me to stop what I was doing and retrieve it for him. It seemed in the first case that he forgot it, while yester day, even though it was gone, out of his sight, he still remembered it and wished it back.
