This report discusses several different approaches that support reforming health care services in developing countries. For some time now, health care services have been supported by government funds. As demands for improving health care services continue to increase additional demands will be placed on governments to respond. This, however, will not be easy. Slow economic growth and record budget deficits in the 1980's have forced reductions in public spending. Alternative approaches to finance health care services are needed. Such possible changes could involve: decentralization of federal government involvement; the promotion of nongovernment involvement; the imposition of user fees; and, establishing health insurance. Finally, the role of the Bank in pursuing new financing strategies is discussed.

January 1998 There is an apparent consensus that the correct health policy in developing countries is public provision of a mix of preventive and simple curative services through low level health workers and facilities. But the strength of this consensus on the primary health care paradigm is in sharp contrast to either the strength of its analytical foundations or its mixed record in practice. Filmer, Hammer, and Pritchett show how the recent empirical and theoretical literature on health policy sheds light on the disappointing experience with the implementation of primary health care. They emphasize the evidence on two weak links between government spending on health and improvements in health status. First, the capability of developing country governments to provide effective services varies widely-so health spending, even on the right services, may lead to little actual provision of services. Second, the net impact of government provision of health services depends on the severity of market failures. Evidence suggests these are the least severe for relatively inexpensive curative services, which often absorb the bulk of primary health care budgets. Government policy in health can more usefully focus directly on mitigating market failures in traditional public health activities and, in more developed settings, failures in the markets for risk mitigation. Addressing poverty requires consideration of a much broader set of policies which may-or may not-include provision of health services. This paper-a product of Poverty and Human Resources, Development Research Group-is part of a larger effort in the group to investigate efficacy in the social sectors. This paper discusses the nature of private sector involvement in health service delivery in developing countries, the potential for expanding it, and areas where health sector policy goals might benefit from greater private participation. It takes the wide and rarely occupied middle ground between the extreme pro-public and pro-private approaches to health care delivery. A simple distinction between the private and public sectors is applied -- anything not done by the government itself is treated as part of the private sector. The area of greatest potential for private sector involvement in developing countries is in hospital care. It would require, however, the development of third party payment systems and a re-evaluation in areas where governments are most heavily involved. This would allow governments to retreat to the simpler and more appropriate tasks of financing and regulating health service delivery. Governments probably cannot extract themselves completely form direct provision of curative care, especially in rural areas. Greater reliance on the private sector to supply remaining government facilities, however can stimulate the development of more diversified private markets and supply lines in rural areas.

This book discusses the significance of local governments in public healthcare systems in developing countries, particularly India. It considers a new model of public health delivery system in the Indian state of Kerala, which is unique in achieving a fairly high level of human and social development with a relatively low level of economic development. Unlike most Indian states, Kerala has devolved the control of health service management to local governments, though the state government still has administrative powers, resulting in a system of dual responsibilities. Transfer of public health control from state government to local governments has seen an increase in the participation of the local community in public health delivery management. Based on a field study conducted in Keralan districts, this book explains the scope for mobilising local resources for the implementation of public health projects under local government, discussing a unique model of co-production between the government and civil society that can improve health services, efficiency and equity, leading to better health outcomes. As such, this study will be of interest to scholars and practitioners in the areas of health, local governance and decentralised planning.
