This book is all about improving the health care of elderly people, which is facing unprecedented challenges in the 1990s. Energies need to be concentrated in three main areas: • the development of high quality care in community settings • the future role of specialized hospital services • the way that long-term care is delivered. All of these are interconnected and success with any of them depends on successful relationships. For example, success in community care requires working interfaces with primary care, the private sector and hospital care. These interfaces are not static but comprise dynamic interactions between people - people in different teams, with different back grounds, training, perspectives and interests. What people do is influenced strongly by the organizations they work for. These inescapable facts lead us to the structure of this book. By concentrating on sectors, people and organizations, we hope to deal with all the important relationships in a coherent way. Yet this book is not intended to be from a service providers' perspective. We have arranged the book in this way so that providers are best equipped to meet the needs of their customers - whether they are patients, resi dents or clients. We know that elderly people may experience dislocation in their care because of frictions and gaps be tween different sectors, disciplines, teams and organizations. To achieve seamless care, there is a need for greater under standing and harmony of purpose and action across the care spectrum.

The United States is currently facing a crisis in health care marked by unsustainable spending and quality that is poor relative to international benchmarks. Yet this is also a critical time of opportunity. Because of its focus on quality of care, the Affordable Care Act is poised to expand access to palliative care services for the sickest, most vulnerable, and therefore most costly, 5% of patients- a small group who nonetheless drive about 50% of all healthcare spending. Palliative care is specialized medical care for people with serious illnesses. It focuses on providing patients with relief from the symptoms, pain, and stress of a serious illness—whatever the diagnosis or stage of illness. The goal is to improve quality of life for both the patient and the family. Research has demonstrated palliative care’s positive impact on health care value. Patients (and family caregivers) receiving palliative care experience improved quality of life, better symptom management, lower rates of depression and anxiety, and improved survival. Because patient and family needs are met, crises are prevented, thereby directly reducing need for emergency department and hospital use and their associated costs. An epiphenomenon of better quality of care, the lower costs associated with palliative care have been observed in multiple studies. The elderly population has specific needs that include coordination of complex chronic disease, diverse social services, and non-health care assistance. However, the Medicare program, the primary insurance program for the elderly, only provides incentives for disjointed, uncoordinated care that results in inappropriate, inefficient, and potentially risky conditions being shouldered by its vulnerable beneficiaries. Case management in integrated delivery systems has been shown to be effective in coordinating elderly clinical and non-clinical needs. These include acute care clinical requirements, chronic disease(s) management, long term care services, and home health as well as end of life services. Medicare, however, does not currently cover case management providers for these purposes. Case management should be a covered service under the traditional Medicare individual provider payment scheme to improve the efficiency of the Medicare program, and, more importantly, to move the Medicare program to a true patient-centered model that respects and empowers the elderly to allow them independence and function for the duration of their lives.
