Scientists and engineers have been involved in medical radiology from the very beginning. At times advances in this field occur at a tremen dously fast pace. Developments in radiological diagnostics have - technologically and medically speaking - focused on morphology. At present, computer-aided tomography (CAT) is at a high point in deve1opment, medical application, and validation. The preconditions for this success were rapid advances in electronics and computer technology - in hardware and in software - and an unexpected cost reduction in these fields; the co operation of various scientific disci plines was also essential. Functional radiological diagnosis has been neglected in part, owing to the emphasis on morphology, but alone the synthesis of morphology and function prornises further advances. Apart from the limited capabilities ofuItrasonic techniques there is no way other than using X-rays to carry out functional studies of organs and their systems through an intact body surface. It is frequently necessary to do further processing and evaluation of image series which have been recorded from the morphological viewpoint. This further picture processing may be of selected points (pixels) in the image, of certain regions of interest (ROI), or of the overall picture. For the measure ment of rapid phenomena, such as the blood flow in the main arteries, high image-frame rates are required, and at the moment these can only be achieved with cinemascopic techniques. For slower processes, other techniques such as videography have some advan tages.

The quantitative analysis of blood flow within central and peripheral blood vessels has attracted more and more interest, for with the rapid developments in vascular surgery and the introduction of percutaneous transluminal angioplasty, it is becom ing increasingly important to be able to measure regional blood flow in man. In clinical radiology, angiography has been used predominantly from the point of view of its morphological applications. However, theoretically angiography may also be regarded as a specific application of indicator-dilution measurements of blood flow. The indicator is contrast medium (CM), the changing concentration of which is re corded by cinematography or video-electronic systems at sites downstream from the point of injection. The curves of density thus obtained correspond to indicator-di lution curves. The blood flow can be calculated from the concentration-time curves ofCM in much the same way as it is estimated from other indicator-dilution curves. In our early work with clinical application of videodensitometric measurement of blood flow in the iliac artery, we found that this method did not offer as high a degree of accuracy as one would have expected from investigations by other authors. We saw that we needed to examine the following problems: 1. We had to investigate whether our conventional X-ray equipment and video densitometer could be viewed as a linear measuring system. To what extent do unavoidable nonlinear changes in the measuring signals influence the blood flow values? 2.

In recent years there have been major advances in the fields of cardiovascular nuclear medicine and cardiac magnetic resonance imaging. In nuclear cardiology more adequate tomographic systems have been designed for routine cardiac use, as well as new or improved quantitative analytic software packages both for planar and tomographic studies implemented on modern state-of-the-art workstations. In addition, artificial intelligence techniques are being applied to these images in attempts to interpret the nuclear studies in a more objective and reproducible manner. This book covers almost every aspect of quantitative cardiovascular nuclear medicine and magnetic resonance imaging. Over the past twenty years, technical advances in coronary arteriography have contributed to our understanding of the pathophysiologic aspects and natural history of coronary artery disease. Probably more than 700. 000 coronary arterio grams are performed annually throughout the world. Usually, these arteriograms are interpreted visually to determine the morphologic extent and severity of coronary artery disease. These subjective determinations, which are hampered . by relatively large intra- and interobserver variations, are used as a basis for critically important therapeutic decisions: Which arteries are to be revascular ized, which lesions are suitable for coronary bypass surgery or for percutaneous coronary angioplasty? With such new procedures and technologies the efficacy of new thera peutic procedures, the effects of vasodilating and constricting drugs, and the results of long-term studies on the regression and progression of atherosclerotic plaque can be determined in an objective and cost-effective manner.
