Now in its fourth edition, Rational Diagnosis and Treatment: Evidence-Based Clinical Decision-Making is a unique book to look at evidence-based medicine and the difficulty of applying evidence from group studies to individual patients. The book analyses the successive stages of the decision process and deals with topics such as the examination of the patient, the reliability of clinical data, the logic of diagnosis, the fallacies of uncontrolled therapeutic experience and the need for randomised clinical trials and meta-analyses. It is the main theme of the book that, whenever possible, clinical decisions must be based on the evidence from clinical research, but the authors also explain the pitfalls of such research and the problems involved in applying evidence from groups of patients to the individual patient. For this new edition, the sections on placebo and meta-analysis and on alternative medicine have been thoroughly updated, and there is more focus on insufficient reporting of harms of interventions. The sections on different research designs describe advantages and limitations, and the increased medicalisation and the effects of cancer screening on health people are noted. A section on academic freedom when clinicians collaborate with industry and ghost authors is added. This essential reference work integrates the science and statistical approach of evidence-based medicine with the art and humanism of medical practice; distinguishing between data, sets of data, knowledge and wisdom, and their application. Such an intellectually challenging book is ideal for both medical students and doctors who require theoretical and practical clinical skills to help ensure that they apply theory in practice.

Excerpt from Treatment of Disease in Children: Including the Outlines of Diagnosis and the Chief Pathological Differences Between Children and Adults It has been my endeavour to prepare for the senior student and practitioner a concise but complete manual of the treat ment of disease in children. I have made one assumption that the reader is already acquainted with a work on general medicine. I should like to have reflected a picture of all contemporary treatment of disease in children. This was my original intention. But the material accumulated apace; and a volume of vast dimensions would have been required. Instead I have endeavoured to give what I thought best: to present a cc-ordinate microcosm for the inco-ordinate chaos. All through the book I have emphasized that treatment I am accustomed to adopt. The Alpha and Omega of all rational treatment is precise Diagnosis. It is rare that in treatment our indiscretion serveth us better than our judgment. The outlines of Diag nosis are given, but Diagnosis is a matter of practical clinical skill and sagacity hardly to be acquired from books. Written specifically for non-infectious disease specialists in both inpatient and outpatient settings, A Rational Approach to Clinical Infectious Diseases provides concise, practical guidance that mimics the decision-making process and reasoning employed by an ID physician. Using clear, understandable language, Dr. Zelalem Temesgen and his esteemed colleagues at the Mayo Clinic present the art and the context of infectious diseases together with the science, helping non-specialists apply a rational approach to the diagnosis and treatment of infectious conditions. Clearly explains the rationale of opting for one particular treatment or length of course over another in order to arrange appropriate management and follow-up. Provides focused ID decision support to questions such as: What diagnostic test should I order? What is the correct antibiotic for this patient/geographical region? Are IV or oral antibiotics most appropriate? How long should the antibiotic course be and when should it be de-escalated? What special considerations should be taken in immunocompromised patients? How often should complex infections be followed up? Uses a succinct, easy-to-read writing style, following a consistent format: Important characteristics/epidemiology; Clinical related data; Rash characteristics; Ancillary diagnostic studies; Treatment; and Other. Provides visual and quick-reference support with dozens of figures and tables throughout the text. Contains invaluable guidance to help non-specialists provide the best care for patients, stem antibiotic misuse and resistance, avoid adverse drug events, and avoid unnecessary costs.

Excerpt from Medical Diagnosis: A Manual of Clinical Methods It is a creditable characteristic of the treatment of disease in the present day that it seeks to proceed on rational principles. Some there may still be who think it enough to give a name to a collective group of symptoms, and treat the disease as they have been told an affection bearing that name should be treated. There may be others who seize upon a few prominent symptoms, and direct their remedies exclusively to these. But every day is, happily, reducing the number of these routine practitioners, and teaching that the true physician is he who seeks thoroughly to investigate the phenomena of the disease, that in this way he may the better arrive at a knowledge of that from which they proceed, and to which, therefore, his treatment should be directed. But this can only be arrived at by a thorough knowledge of every change which disease produces in the body, and by a clear conception of what that change imports. This constitutes the science of Diagnosis, and, without accurate diagnosis, there can be no rational treatment. The signs and symptoms of disease are changes produced in the animal economy, which are cognoscible by our senses - some by one, others by another; while to assist these senses we call in the aid of instruments which extend their range or increase their power, and of the various analytical processes which the science of Chemistry places at our disposal. In the following pages an attempt has been made to describe these signs and symptoms of disease, and to show what is their value from a diagnostic point of view.
