Oxidative stress induced by hyperglycemia is a key factor in the development and progression of diabetes and its vascular complications. Dietary polyphenols have received enormous attention because their consumption has been associated with lower rates of diabetes and cardiovascular diseases. These compounds are of plant origin, and are abundant in fruit, vegetables, chocolate, and nuts, as well as in beverages such as tea, coffee, wine, and soy milk. They can be divided into at least ten separate classes, four of which are important in the human diet: phenolic acids, flavonoids, stilbenes, and lignans. A number of in vitro and in vivo studies support the effect of polyphenols on glucose metabolism, diabetes risk, and diabetic impairments in the nitric oxide-mediated endothelial progenitor cell mobilization and homing. This chapter intends to review the current knowledge on polyphenols, oxidative stress, and vascular damage in diabetes, focusing on the most important and recent advances and challenges for future research.

Since endothelial progenitor cells (EPCs) were first described in 1997, there has been significant debate surrounding their definition and roles; but also agreement in their potential to develop as biomarkers and cytotherapies. The endothelium, a monolayer of endothelial cells, constitutes the inner cellular lining of the blood vessels (arteries, veins and capillaries) and the lymphatic system, and therefore is in direct contact with the blood/lymph and the circulating cells. The endothelium is a major player in the control of blood fluidity, platelet aggregation and vascular tone, a major actor in the regulation of immunology, inflammation and angiogenesis, and an important metabolizing and an endocrine organ. Endothelial cells controls vascular tone, and thereby blood flow, by synthesizing and releasing relaxing and contracting factors such as nitric oxide, metabolites of arachidonic acid via the cyclooxygenases, lipoxygenases and cytochrome P450 pathways, various peptides (endothelin, urotensin, CNP, adrenomedullin, etc.), adenosine, purines, reactive oxygen species and so on. Additionally, endothelial ectoenzymes are required steps in the generation of vasoactive hormones such as angiotensin II. An endothelial dysfunction linked to an imbalance in the synthesis and/or the release of these various endothelial factors may explain the initiation of cardiovascular pathologies (from hypertension to atherosclerosis) or their development and perpetuation.
