There have been significant advances in the treatment of sarcomas in the past several years. Further, different clini cal treatment programs are being advocated in different areas including surgery alone, surgery with preoperative or post operative chemotherapy, surgery with different radiotherapy modalities, with each investigator espousing his own treat ment program. On the other side, there is the question of whether these treatment programs are offering better results or whether the natural history of sarcomas has changed. The International Symposium on Sarcomas was held at Innisbrook Resort, Tarpon Springs, Florida, October 8-10, 1987. This was the first international symposium to date involving all of the disciplines treating sarcomas including pathologists, orthopaedic surgeons, general surgeons, medical oncologists, pediatric oncologists, and radiation oncol ogists. The Symposium brought together a number of special ists working in the clinical field of sarcomas for a presen tation of their specific treatment programs and their results. The presentations were followed by panel discussions to stimulate educational debate as to the different forms of treatment for sarcomas and to formulate some conformity in control of disease, control of spread, and ultimate function for the patient. Where do you begin to look for a recent, authoritative article on the diagnosis or management of a particular malignancy? The few general oncology textbooks are generally out of date. During the past decade, the prognosis of bone and soft tissue tumors has improved considerably. This progress has not only been due to improved therapeutics and diagnostic methods but also due to a team approach now universally adopted. Comparison of results, possible today, were hampered in the past by lack of a clinically relevant staging system and definitive treatment proto cols. The complexity of the therapeutic management of bone tumors re quires a constant exposure to tumors, highly skilled diagnostic tech niques, and specialized expertise. It is therefore not surprising to learn that small and medium-sized hospitals embarking on the diagnosis and treatment of bone and soft tissue tumors are generally less success ful than better equipped major centers. This fact has already been recognized by most physicians and surgeons who now refer their patients to specialized oncologic centers. Nevertheless, they all have to know the present state of the art. This symposium was therefore mostly addressed to them. This international symposium, organized by the Division of Orthopaedic Surgery, University of Ottawa, and cosponsored by the Association des Chirurgiens Orthopedistes du Quebec, was held in Ottawa from May 12-14, 1983. Each of the 40 presentations clearly demonstrates that disregard for proper staging before initiation of treatment and omission of a clearly defined treatment protocol is definitely unacceptable in view of the knowledge we have acquired.

Sarcomas represent a group of rare tumors which arise from the largest tissue compartment of the body, but account only for about 1 % of all malignancies. Among this group of tumors, soft tissue sarcomas in adults are, in addition to osteosarcomas, the most important. In the diagnostic workup of soft tissue sarcomas modern radiological investigation methods such as CT and MRI techniques are being critically evaluated. While it is widely accepted that the conventional, stained microscopic specimen is still the cornerstone for properly planned treatment stra tegies, immunohistochemistry has come to play an indis pensable role in accurate classification. Reviewing the surgical techniques, the treatment of musculo-skeletal malignancies, especially of soft tissue sarcomas, has changed considerably over the past decades. While amputation used to be the surgical treatment of choice limb-saving procedures have now become an im portant therapeutic modality in treating such lesions, particularly in combination with percutaneous or interstitial radiotherapy. Despite adequate local treatment, many patients with soft tissue sarcomas will ultimately develop metastatic disease, usually in the lungs. For this reason, effective ad juvant systemic treatment should be given simultaneously with or soon after treatment of local disease. The results of several randomized trials evaluating adjuvant chemotherapy have been reported in the meanwhile, indicating im proved, disease-free survival with a trend towards improved overall survival.
