Background: The U.S. Bureau of Labor and Statistics (2016) reports that employment of registered nurses (RN) will increase by 16%, resulting in 1.2 million vacancies in the next decade. A recent survey by the National Council of State Boards of Nursing (2013) reports 55% of working RN personnel are 50 years of age or older and will retire within the next 10 to 15 years. This complex problem of demand and supply of nurses is affecting all areas of nursing, especially critical care nursing. The shortage of nurses in critical care is a significant concern because this area requires a high number of educated and specialized nurses. There is minimum research on ICU nurse retention, as most studies have concentrated on the general nursing population. Objective: This qualitative descriptive study explores the nurse managers’ perspective in the following areas: (1) identifying factors relevant to retention (personal, occupational, organizational); (2) understanding current programs and practices focused on retention of critical care nurses; and, (3) exploring plans and directions for improved nurse retention. Methods: Semi-structured interviews were conducted with a convenience sample of five ICU nurse managers from 4 different hospitals in Northern California. Result: This study highlights the importance of best practices such as leadership practices, adequate staffing, resources, autonomy, a healthy work environment, interprofessional collaboration, salaries and incentives in retaining critical care nurses. The three programs supporting critical care nurse retention are orientation, preceptorship, and nurse residency. Relevance to Nursing: The findings will serve as a basis for larger-scale research to establish best practices for retention of critical care nurses. This small study may give some insights to nurse leaders to find strategies which may improve patient outcome, patient satisfaction, job satisfaction, and lower costs.
