There is a wide variation in the clinical syndromes and in the severity of respiratory tract infections. This diversity is only partly accounted for by the large number of infecting agents, because a particular microorganism can itself result in different illnesses. The age, fitness and state of immunity of the patient is of great relevance, and the clinical features of a particular infection vary accordingly. In terms of incidence of infection the acute virus infections are far in the lead. Respiratory infections can be broadly classified into acute and chronic. The acute infections are generally due to bacteria, viruses, rickettsiae and mycoplasma. Chronic infection is either bacterial, mycobacterial, fungal or parasitic in origin. Protection Against Infection There are three methods by which man can protect himself against infections of the respiratory tract-mechanical barriers, phago cytic cells and the immune response. Mechanical Barriers The cough reflex protects the lung against inhaled irritant sub stances and larger particles. Smaller particles will be trapped by the layer of mucus which covers the respiratory tract. The ciliated epithelium maintains a constant upward flow of the mucus and via this mucociliary escalator expels particles from the respiratory tract. 2 Infections Infection can occur when these mechanical barriers are altered, and the defect in the mucociliary lining accounts for the increased susceptibility to infection in chronic bronchitis and bronchiectasis.

The book will be organised by presentation rather than causative organism, a differentiating feature from the existing competition, and will be divided into four parts - 'General Issues', 'Community-acquired Infections', 'Nosocomial Infections' and 'Respiratory Infection in Immunocompromised Patients'. Each chapter will conclude with a summary highlighting practical 'take-home' messages regarding patient management for the reader. It will be highly illustrated, including colour plates where appropriate. The principal market will be pulmonologists with a specialist interest in this area and infectious disease specialists, but that the text will also be of interest to clinical researchers and a useful library reference.

The lower respiratory tract infections are more serious than the upper respiratory tract infections. Some respiratory tract infections are sinusitis, tonsillitis, pharyngitis, influenza, bronchitis and pneumonia. Human orthopneumovirus is a major cause of lower respiratory tract infections. It generally causes conditions like bronchiolitis and pneumonia. These can be diagnosed through physical examination, skin monitoring to check for hypoxemia, chest X-rays, blood tests, and lab test of respiratory secretions. This book explores all the important aspects of respiratory infections in the present day scenario. It includes some of the vital pieces of work being conducted across the world, on various topics related to the diagnosis and treatment of respiratory infections. Respiratory infections are among the most common causes of morbidity and mortality worldwide. These infections present a special challenge to physicians for several reasons, including of the recent disasters caused by severe acute respiratory syndrome (SARS) and bird and swine influenza, the increase in viral and bacterial resistance to currently available anti-microbial drugs, the increased frequency of new viral lung infections in clinical practice, and the worldwide emergence of deadly drug-resistant forms of TB. For these reasons, it is important for infectious disease specialists to have an overview of emerging respiratory infections.

A step-by-step guide to the correct management of respiratory infections in infants and young children. Addressed to staff in first-level health facilities, the book explains how to assess a child, recognize symptoms, classify the illness, and provide appropriate treatment, whether involving immediate referral to hospital, treatment with antibiotics at home, or simple home care. Throughout, a special effort is made to help staff distinguish the few, very sick children from the many others whose infections can be safely treated at home. The manual also aims to help prevent deaths from pneumonia and other severe illnesses by offering abundant advice on the recognition and urgent management of danger signs. The manual has two parts, each based on the use of a quick-reference case management chart, reproduced in a foldout version. Part one covers the management of pneumonia, wheezing, coughs and colds. Advice on how to collect essential information about the signs of respiratory problems is followed by a description of the steps involved in classifying the illness, determining the degree of severity, and identifying the appropriate treatment. Part one concludes with detailed treatment instructions, including how to give an antibiotic and treat fever and wheezing. Part two explains how to assess, classify, and treat the child with an ear problem or sore throat. Treatment instructions include advice on how to dry the ear by wicking, give an antibiotic, and manage streptococcal sore throat. Further practical information is provided in a series of annexes, which explain how to refer a child to hospital, what to do when referral is not feasible, how to use a rapid-acting bronchodilator, and how to determine whether complaints and symptoms are due to measles, pertussis, or diphtheria.
