Concerns about the negative impact of the "brain drain" of health professionals from Africa have led to a dominant narrative in which those who migrate are a permanent, and costly, loss to the country of origin and a permanent, and valuable, gain for the country of destination. In the new world of transnationalism, a global skills market, and greatly increased mobility by health professionals, it is unlikely that the traditional permanent-exodus model of the brain drain narrative adequately captures all forms of migration by South African doctors. This report first examines the temporary employment opportunities for South African doctors in countries such as the United Kingdom, Ireland, Canada, and Australia. This report draws two major conclusions: first, the dominant brain drain narrative overlooks the complex nature of South African physician migration and ignores the fact that a significant number of doctors have temporary employment experience outside the country. Second, it suggests that temporary employment overseas increases the chances of permanent emigration later.

Many of America's greatest artists, scientists, investors, educators, and entrepreneurs have come from abroad. Rather than suffering from the "brain drain" of talented and educated individuals emigrating, the United States has benefited greatly over the years from the "brain gain" of immigration. These gifted immigrants have engineered advances in energy, information technology, international commerce, sports, arts, and culture. To stay competitive, the United States must institute more of an open-door policy to attract unique talents from other nations. Yet Americans resist such a policy despite their own immigrant histories and the substantial social, economic, intellectual, and cultural benefits of welcoming newcomers. Why? In Brain Gain, Darrell West asserts that perception or "vision" is one reason reform in immigration policy is so politically difficult. Public discourse tends to emphasize the perceived negatives. Fear too often trumps optimism and reason. And democracy is messy, with policy principles that are often difficult to reconcile. The seeming irrationality of U.S. immigration policy arises from a variety of thorny and interrelated factors: particularistic politics and fragmented institutions, public concern regarding education and employment, anger over taxes and social services, and ambivalence about national identity, culture, and language. Add to that stew a myopic (or worse) press, persistent fears of terrorism, and the difficulties of implementing border enforcement and legal justice. West prescribes a series of reforms that will put America on a better course and enhance its long-term social and economic prosperity. In contrast to other studies on identity, this book takes its point of departure in the complexities that characterize and shape both individuals and societies – past and present. Its chapters challenge demarcated fields of study and conceptions of identity as gender, identity as functional disability, identity as race, and identity as, or based upon language groupings. The contributions take a social practices perspective in their exploration of the performance, living and doing of identity positions across time and space. Many of the contributions take an intersectional stance and the majority report upon empirically driven studies that examine the ways in which micro-level analyses of naturally occurring human communication contribute to our understanding of identification processes.
