Fachhochschule Frankfurt am Main, Sprache: Deutsch, Abstract: Zu Beginn der Beschäftigung mit dem Themengebiet Risikomanagement im Krankenhaus stellt sich die Frage nach der Notwendigkeit. Das Krankenhaus als System besteht, um Leiden zu lindern, zu verbessern, wenn nicht gar zu heilen. Die Realität sieht leider anders aus. Das Gesundheitswesen ist nicht so sicher wie es sein sollte. Im Jahr 2000 wurde der Bericht „To Err is Human“ durch das Institute of Medicine der US-amerikanischen National Academy of Sciences veröffentlicht. Dieser erregte international großes Aufsehen und rückte das Thema „Medizinische Risiken, Fehler und Patientensicherheit“ zunehmend in den Blickpunkt des fach-wissenschaftlichen Interesses. „To Err is Human“ bezieht sich u.a. auf zwei große Studien in New York (1984) und in den Staaten Colorado und Utah (1992). Die Auswertung von 30.000 (New York) bzw. 15.000 (Colorado/Utah) stationären Fällen ergab Komplikationsraten von 2,9 und 3,7 Prozent. 6,6 bzw. 13,6 Prozent dieser Komplikationen verliefen dabei tödlich. In beiden Studien konnten in über 50 Prozent der Fälle medizinische Behandlungsfehler und in über 25 Prozent der Fälle Nachlässigkeiten bzw. Unterlassungen als Ursachen definiert werden. Rechnet man diese Zahlen auf die 33,6 Millionen Behandlungen in den USA im Jahr 1997 hoch, wie dies in der Veröffentlichung getan wurde, sterben jährlich 44.000 - 98.000 US-Amerikaner an den Folgen medizinischer Fehler. Das bedeutet: auch bei vorsichtigen Hochrechnungen rangieren medizinische Behandlungsfehler auf Platz 8 der wichtigsten Todesursachen in den USA, noch vor Autounfällen (43.458), Brustkrebs (42,297) oder AIDS (16.516) . Die verursachten nationalen Gesamtkosten liegen nach Schätzung von Kohn/Corrigan/Donaldson zwischen 17 und 29 Milliarden US-Dollar. [...]
