The categorical risk-levels are based on a numeric score classification and include Imminent Risk, Moderate/High Risk, and Low Risk. With each risk level, suggested corresponding staff action steps are provided to guide the intervention in consideration of existing liability (Tort-Law) and documentation. Since 2009 and 2023 (2nd Edition), the RISK2 Manual walks you through the evaluation process while providing additional techniques and troubleshooting ideas. After receiving rigorous psychometric analysis, the RISK2 is one of just a few validated instruments to achieve strong internal consistency, validity, reliability, and predictability in the youngest children. The content of the RISK2 and RISK2 Manual are based on the research findings of over 5,949 ten-item pre/post Suicide Prevention Knowledge Surveys, 4,057 school mental health referrals, and the dissection of over 1,404 suicide assessments. Working within school and community mental health settings since 1991, I've been afforded the opportunity to listen, observe, and experience the frustrations of school staff and mental health practitioners who would often find themselves in difficult situations without having the tools, support, training, and resources to properly evaluate suicide risk. This led to the development of the Risk Identification Suicide Kit2 (RISK2) becoming an empirically based multileveled approach to suicide risk assessment from 5 to 24 years of age. The RISK2 is not a screener but used after a youth has been identified as having suicidal thoughts. Since 2009, the RISK2 makes this process easier while providing personalized intervention care and risk management using the RISK2 Safety Plan. The RISK2 is designed for schools and mental health practitioners to confidently provide an easy-flowing structured and guided evaluation of suicidal thoughts, behaviors, and supportive risk management. The content is based on the results of 5,949 ten-item pre/post Suicide Prevention Knowledge Surveys, 4,057 school mental health referrals, and the dissection of 1,404 suicide risk assessments. This research identified and cited key indicators used to measure current suicidal ideation, identified protective factors, suicidal behaviors, and correlational estimates of acting on existing suicidal thoughts that were interrelated to the classification of the youth's risk level. The RISK2 received multiple independent psychometric analysis achieving reliability, validity, and excellent predictability values based on 1,404 participants. The RISK2 includes eight sections including demographic data, contact information, guided verbal response self-report interview, an ambivalence status assessment of protective factors and reasons to die/live, a 12-item suicide specific questionnaire, mental status exam, documentation, risk level - Imminent Risk, Moderate/High Risk, & Low Risk, parent contact notes, and recommended action steps. With each risk level, suggested corresponding staff action steps are provided to guide the intervention in consideration of existing liability and documentation. The vision of the RISK2 is to support, empower, and guide practitioners in the suicide assessment process while leading the youth and family towards a path of recovery and a desire to choose living. 25 RISK2 Assessments are included per pack.
