Das Leben ist endlich und geht früher oder später unweigerlich auf den Tod zu. Bis in die jüngste Vergangenheit war der Mensch dem Sterben und dem bevorstehenden Tod ohnmächtig ausgesetzt. Es war ein fremd verfügtes Schicksal, gegen das die Betroffenen nichts ausrichten konnten. Dies hat sich radikal geändert. Heute stehen der Medizin praktisch in jeder Situation lebenserhaltende und -verlängernde Möglichkeiten zur Verfügung. Das bringt zwar einen unbestreitbaren Freiheitsgewinn mit sich, bürdet den Sterbenden aber zugleich eine moralische Verantwortung für ihr Sterben auf. Das bedeutet, dass jede und jeder sich mit dem Thema "Selbstbestimmtes Sterben" auseinandersetzen sollte. Denn es müssen am Lebensende oft schwierige Entscheidungen getroffen werden: Sollen medizinischen Eingriffe gemacht werden und mit welchen möglichen Folgen ist zu rechnen? Soll die Ärzteschaft alles ihr Mögliches tun, um Leben zu verlängern, auch wenn die Lebensqualität darunter leidet? Oder bevorzugt die Patientin Palliative Care, bei der auf heilende medizinische Massnahmen verzichtet wird und mit lindernden Massnahmen der Sterbeprozess erleichtert werden soll? Jüngste Studien ergeben, dass in der Schweiz in rund 60 Prozent aller Fälle Menschen erst sterben, nachdem entsprechende medizinische Lebensende-Entscheidungen gefällt worden sind, also Entscheide, dass man sie sterben lässt, obwohl man sie noch weiter durch entsprechende Massnahmen am Leben erhalten könnte. Selbstbestimmtes Sterben ist heute folglich der neue Normalfall des Sterbens geworden. Er stellt sich täglich bei Betroffenen, ihren Angehörigen und den behandelnden Personen in Spitälern, Heimen und Hospizen. Trotzdem ist in der gegenwärtigen Situation das selbstbestimmte Sterben ein Thema, das oft wie ein Tabu behandelt wird und viel zu wenig Aufmerksamkeit erhält. Auch gibt es dazu nur wenig Literatur. Heinz Rüegger und Roland Kunz möchten mit ihrem Buch "Selbstbestimmtes Sterben - Zwischen Freiheit, Verantwortung und Überforderung" die Debatte für ein breiteres Publikum anstossen. Sie skizzieren, wie es zur heutigen Situation gekommen ist, was sich dadurch für die Ärzteschaft und die sterbenden Personen geändert hat. Sie diskutieren die Frage, was Selbstbestimmung heisst, welche Schwierigkeiten beim Entscheiden über Leben und Tod entstehen und machen sich Gedanken dazu, ob man Sterben lernen kann. Und selbstverständlich werden auch die Rahmenbedingungen des selbstbestimmten Entscheidens beleuchtet: u.a. die rechtlichen Bestimmungen, die ärztliche Information und Kommunikation und der Druck durch das Umfeld der Betroffenen. Das Buch richtet sich an eine breite Leserschaft, an betroffene Personen und ihre Angehörigen und insbesondere auch an die Ärzteschaft, Pflegefachleute, GeriaterInnen und weitere ExpertInnen aus Gesundheitsberufen.
