In this book the lectures of the first course of the International School of Cardiology at Ettore Majorana are presented. It is difficult to reflect in a publication of this kind the atmosphere and spirit of this postgraduate course. Moreover, the beautiful scenery of Erice and its surroundings, celebrated by ancient Greek poets, can never be described by the editors of this book. The purpo~e of this course was to deepen our knowledge accumulated todate on the subjects of non-invasive cardio circulatory assessment and the analysis of the comprehensive approach to cardiac rehabilitation. The clinical value of exercise testing, echo-cardiography, scintigraphy, systolic time intervals have been discussed, as well as the tasks of exercise training, surgery and drugs in the secondary prevention of coronary heart disease. The problems of arrhythmias and the prevention of sudden death have been touched on, as well as early m~bilization after myocardial infarction, the use of digoxin and beta blockers and finally, the controversies in cardiac rehabilitation.

The aim of the 2nd Course of the International School of Cardi ology at "Ettore Majorana" was the discussion, scientific analysis, and critical appraisal of primary and secondary prevention in cardi ology, especially concerning the coronary artery disease. The predictive value of different risk factors, the problem of heredity, sociological and psychological perspectives, the impact of antiarrhythmic therapy, and the prevention of sudden death were only some of the many topics discussed. Others, such as physical activity in primary and secondary prevention, the role of coronary bypass surgery in decreasing the mortality rate, the effects of drugs and comprehensive intervention programmes, as well as a critical appraisal of therapy in cardiac failure were given special attention. In organizing this second Course, it was again a privilege and a challenge for the programme directors to conduct a scientific event which, as was demonstrated at the first course, represented an original and new approach to teaching. The illustrious international faculty, composed of well known scientists and experts in their fields, has contributed to the high academic level and quality of the lectures and the following discussions. In our opinion, this successful undertaking would not have achieved its purpose and originality without the beautiful surround ing scenery and tranquility of Erice. This book is a comprehensive overview of electrocardiography and the major effects of current cardiac pharmacological therapy on electrocardiography. The text is based on work presented at the International Symposium on Non-invasive Cardiovascular Diagnosis and Therapy, held in May, 1987 at the University of Cologne. The theme of the book is to review, in broad clinical perspective the current state-of-the-art of electrocardiography as it pertains to standard electrocar diograms, exercise testing, ambulatory electrocardiography, electrocardiographic telemetry, and high resolution electrocardiography. Furthermore, advance!, in cardiac drug therapy in relation to diuretics, beta blocking drugs, antiarrhythmic agents and thrombolytic agents are reviewed. The emphasis of the conference and this book is to review the clinical state-of-the-art information and applications in this regard. In the initial section on electrocardiography, Dr. Spodick reviews our present day physiologic and pathophysiologic understanding of systolic time intervals, and how they are affected by a variety of cardiac disease states and pharmacologic agents. Dr. Ellestad examines problems and provides pragmatic tips on exercise testing in the diagnosis of coronary artery disease, and advances in exercise scores and computer analysis. Dr. Graboys reviews the value of exercise testing in the diagnosis and management of patients with serious ventricular arrhythmias. Dr. Kellermann presents the complimentary role that exercise testing plays in com prehensive follow-up therapy of the cardiac patient, and the use of exercise for work and physical training. Exercise testing is widely used all over the world to assess functional capacity in athletes, healthy subjects and patients. According to recent surveys, the interest in ergometry is still growing in almost all fields of medicine, especially in private practice. Furthermore, there has been an exponential growth in the number of publications on exercise testing in the last years. Several consensus and task force conferences have dealt with exercise testing and published recommendations on standardization and guidelines in ergometry. These factors have, in combination, initiated an upsurge in research and clinical use of exercise testing. At the 6th International Seminar on Ergometry the latest findings and advances in ergometry were discussed. Reviews and results of the congress covering a wide range of features in exercise testing are presented in this book. The editors hope that this book will make a substantial contribution to our knowledge regarding exercise testing and will help physicians to appropriately evaluate exercise testing in healthy and diseased subjects. This is the 3rd Course that has been organized at the presti gious E. Majorana Centre. The choice of the theme is not a casual one. In fact after rehabilitation and prevention which were the topics discussed in the previous courses of 1979 and 1982, this Course deals with subjects more deeply connected with the mechanisms of cardiac function and the pathological aspects that under this point of view characterize many specific diseases. The rapid development in the application of technology to the problems of Cardiological diagnosis has made it possible to study ventricular function in the evaluation of patients with apparent or suspected heart disease. Knowing that we are facing one of the most complex subjects in modern cardiology we wish to arouse the interest of researchers anrl cardiologists by comparing different experiences with the aim of giving an overall survey of the problems. The program has been divided into 4 chapters: 1. Systolic and diastolic ventricular function. 2. Specific studies of ventricular function using invasive and non invasive techniques. 3. Diseases with altered ventricular function. 4. Pharmacological manipulation of ventricular function. Our aim is not only to stimulate useful discussions in the faculty but also among the participants. I hope that we can achieve this. This is the first course devoted to bioelectrochemistry held within the frame work of the International School of Biophysics. Although this branch of scientific research is already about two centuries old, as a truly independent one it has been in a stage of lively development since only a few decades ago and this is why a first course at the E. Majorana Center was devoted to it. Since bioelectrochemistry consists of many sub-fields, it is impossible to include, even superficially, all of them in a short course lasting just a week, and therefore the chapter of redox-reactions was chosen for this first course as being most general in character. But even restricting the course to redox-reactions, only a few subjects could be included and therefore the choice among them was made considering the most general guidelines that could serve as a basis for the further study of individual problems. In this way we hope to give a sound basis to the study of and to stimulate further interest in this branch of both biological and physical chemistry. This dual interdisciplinary approach is, on the other hand, unavoidable if a more rigorous and logical attack on biological problems in living bodies is to be carried ahead. Four years ago when the first European Symposium on the re lationship between alterations of blood clotting mechanisms and atherosclerosis was organized, we asked ourselves which would be the best way to obtain both scientific and practical contributions. We have been interested in cardiovascular diseases for several years now and have therefore focused our attention on the "container" (Le. the blood vessel) rather than on the "contents" (Le. the various components of blood) as considered only from a haemodynamic point of view. In recent years correlations were found between alterations of vascular wall and alteration of coagulative, fibrinolytic, and plate let factors as well as of haemorheological phenomena in the thrombo genic evolution of atherosclerotic lesions. A close cooperation between cardiologists and workers interested in atherosclerosis and thrombosis is therefore necessary. We think that the most appropriate approach to the various problems concerning correlations between thrombogenic and atherosclerotic lesions is co-operation between experts in these different fields of research. We thus decided to organize the 2nd Symposium taking into account the great progress achieved in this field during the past few years, and hope that discussions on diagnostic and therapeutical perspec' tives will yield useful elements both for the cardiologist and for the cardiologist and for the general practitioner.
