Introduction: Sensorimotor training (SMT) is used in rehabilitation to improve muscle reflexes. In daily practice, two types of devices were used. Firstly, active therapy devices, such as whole-body vibration devices (WBV), and secondly passive therapy devices, such as foam mats, balance boards, etc. were used. Purpose: The question that arises is if these devices could produce the necessary stimuli for the sensorimotor system in order to obtain the desired motor response. Method: A narrative literature review was carried out. Results: Investigations described that passive therapy devices produce primary weak stimuli below 3 Hz. Active treatment devices could produce stimuli between 1 Hz to 60 Hz. Conclusion: The movement control and movement coordination are subject to the functional capability of the sensorimotor system. This includes the recording, its forwarding and the signal transmission. In daily life, a muscle response must occur within a certain time frame, with the aim to stabilizing a joint or to keep the body in balance. For example, the quadriceps muscle must contract during walking within 125 to 200 ms or during stairs climbing within 100 to 150 ms. Implemented to treatment modalities, a frequency range between 3 and 6 Hz should be used.

This book explains to physical therapists, occupational therapists, speech therapists, interested physicians and clinicians the theoretical and practical applications of single- to multi-channel functional electrical stimulation for a wide range of neurological symptoms. The targeted electrical stimulation of several muscle groups, timed to each other, can initiate and support a complete movement sequence and thus improve motor learning. Renowned experts from research and practice have compiled numerous application examples based on the available evidence in this comprehensive form for the first time. In addition, the reader will find exciting and informative contributions to the current study situation and effectiveness analyses. The text is enriched by videos on EMG-triggered functional multichannel electrical stimulation, stimulation of muscle groups in lower motor neuron lesions, and FES therapy approaches for dysarthria and swallowing disorders. From the contents: - FES in lesions of the upper motoneuron, lesion of the lower motoneuron and mixed forms - Combination of FES with mirror therapy and botulinum toxin - Motor learning, neuroplasticity, ICF-based goal setting and the use in home-based therapy - Basic principles, current parameters and their implications - FES in rehabilitation of facial paralysis, unilateral vocal fold paralysis, dysarthria and dysphagia, and neuro-urological deficits

Peripheral nerve injuries (PNIs) by trauma are the most common neuronal injury in civilian and military populations and significantly burden health care systems. Mammals (including humans) with PNIs experience: (1) immediate loss of sensory and motor functions mediated by the denervated target tissues; (2) rapid (3-7d) Wallerian Degeneration (WD) of severed distal axonal segments; and, (3) slow (~1mm/day) regeneration by naturally occurring axonal outgrowths from surviving, severed proximal stumps that produce poor (if any) functional recovery because of slow axonal regeneration for long distances and lack of axonal guidance. Denervated muscle fibers and sensory organs often atrophy before any re-innervation can occur.

Stroke Rehabilitation: Insights from Neuroscience and Imaging informs and challenges neurologists, rehabilitation therapists, imagers, and stroke specialists to adopt more restorative and scientific approaches to stroke rehabilitation based on new evidence from neuroscience and neuroimaging literatures. The fields of cognitive neuroscience and neuroimaging are advancing rapidly and providing new insights into human behavior and learning. Similarly, improved knowledge of how the brain processes information after injury and recovers over time is providing new perspectives on what can be achieved through rehabilitation. Stroke Rehabilitation explores the potential to shape and maximize neural plastic changes in the brain after stroke from a multimodal perspective. Active skill based learning is identified as a central element of a restorative approach to rehabilitation. The evidence behind core learning principles as well as specific learning strategies that have been applied to retrain lost functions of movement, sensation, cognition and language are also discussed. Current interventions are evaluated relative to this knowledge base and examples are given of how active learning principles have been successfully applied in specific interventions. The benefits and evidence behind enriched environments is reviewed with examples of potential application in stroke rehabilitation. The capacity of adjunctive therapies, such as transcranial magnetic stimulation, to modulate receptivity of the damaged brain to benefit from behavioral interventions is also discussed in the context of this multimodal approach. Focusing on new insights from neuroscience and imaging, the book explores the potential to tailor interventions to the individual based on viable brain networks.
