La sepsis es una condición compleja que se da por una respuesta inflamatoria sistémica frente a un agente infeccioso que amenaza la vida del paciente por compromiso orgánico; es considerada como uno de los mayores problemas en salud pública, debido a su alta incidencia, de aproximadamente 48 millones de casos al año, y elevada mortalidad, cercana a 1 de cada 4 casos en países desarrollados. Se estima que la mortalidad es mayor en países en desarrollo, aproximadamente 11 millones (19.7 %) alrededor del mundo, con tasas promedio del 52.8 % en pacientes con shock séptico. En nuestra región, estudios realizados en Brasil reflejan una mortalidad de hasta el 57 %, y en Colombia se encuentra en-tre el 26 % y el 27 % para sepsis severa y el 46 % y el 45.1 % para choque séptico. En este libro, los autores presentan una estrategia de abordaje integral del paciente séptico, partiendo de una explicación fisiopa-tológica de la enfermedad y revisando cuatro pilares para reducir la mortalidad: reconocimiento temprano, diagnóstico enfocado y molecular, tratamiento empírico y dirigido y seguimiento oportuno y costo-consciente. Se espera que este texto sea de utilidad en la práctica clínica diaria.
