Sleep is a fundamental physiological feature experienced by all known mammalian, and most non-mammalian, species. Underscoring its importance is the wide array of neural and cellular processes that have evolved to govern when and how it occurs, its duration, sequence of phases, and the influence it exerts on numerous other brain functions. This book takes up the growing prevalence of sleep disorders affecting these processes and the panorama of pharmaceutical tools that have evolved for their medical care. Its wide-ranging discussion promises not only recent updates on their clinical management but a contemporary window into sleep’s cross-cutting relevance for the many neurological dysfunctions now known to associate with sleep disturbances.

From the emergence of clinical sleep medicine marked by the establishment of the harbinger Stanford Sleep Disorders Clinic in the mid 1970s, offspring sleep dis- ders clinics and centers have grown exponentially with the recognition of the unmet diagnostic and treatment needs of the reservoir of patients suffering from sy- toms of what are now recognized and classi?ed as the nosology of human sleep disorders. Important in the growing armamentarium of treatment options for the sleep practitioner are both traditional and newer pharmacological agents, including over-the-counter, non-traditional, and prescription types, that are all used to treat, sometimes adjunctively, most clinically recognized sleep disorders. Although there are numerous academic treatises and reviews dealing with in- vidual treatment alternatives for the diversity of recognized sleep disorders, no one comprehensive resource, extant, has dealt with pharmacological treatment options and strategies for the major human sleep disorders associated with a panoply of symptomatic conditions. The present volume and its series of chapters individually focusing on a range of human conditions, from pediatric sleep disorders to sle- relateddisordersofindividualssufferingfromAlzheimer’sdementia, uniquelycover the wide range of human medical conditions amenable to thoughtfully sleep-related applied drug therapy. The Editors have brought together a superb group of internationally respected sleep clinicians, and researchers, that provide state-of-the-art analysis of the current basic and clinical perspective regarding the most common sleep disorders that are amenable to pharmacological treatment. In each chapter the authors outline a th- oughhistoricalbackgroundoftheparticulardisorderandreviewthebasicpre-clinical studies leading to current treatment options.

Sleep plays an important role in the history of the neurosciences. On Easter Monday in 1920, Otto Loewi was awakened in the night by a dream in which he conceived of neurotransmitters communicating across the synapse. He quickly made notes, but in the morning he could not understand his scribbles. The following night, the dream came again. He wrote down his thoughts more carefully and, the next day, conducted the crucial experiment that launched modern neu rophysiology (Koelle, 1986). Since the beginning of the modern era of sleep research in the 1950s, we have used the principles of neurotransmission to explore the regulation of sleep. Without resorting excessively to comments on blind men and elephants, however, it is fair to say that the phenomena of sleep and waking can be approached from many perspectives. Among other things, sleep is a process that can be described electri cally, an experience that so far defies physiological measurements, and a social behavior. In this book, I have tried to describe the physiology and pharmacology of sleep (Part I) and to relate them to clinical sleep disorders (Part II). Having neither the skill nor the grandiosity of Rous seau, I have made no attempt to write an encyclopedia of all that is known on the subject. Rather, I think of this book as more of a snap shot, giving a picture of where we are, and it is hoped, a history of how we got here.
