Recent policy debates about the future of Social Security and Medicare have been dominated by the issue of personal versus societal responsibility. Significant changes now being considered would increase the level of privatization and tilt these programs away from defined benefit toward defined contribution models. This annual volume of the National Academy of Social Insurance examines the concept of introducing choice into the Social Security and Medicare programs, how it would be defined and structured, and what sort of safeguards would be needed to protect program participants. The ideas, from representatives of the public and private sectors, range from ¡°tinkering¡± to ¡°overhauling¡± the programs to make them more responsive and cost-effective. The contributors provide an overview of the history and fundamental values of social insurance, discuss options for reforming Social Security and Medicare, review the benefits and drawbacks of expanding choice options, explore the types of mechanisms needed to protect consumers if market-based reforms are adopted, and address the political likelihood of Social Security and Medicare reforms. The essays in this volume give parameters to the debate over the future of Social Security and Medicare, and reflect the range and diversity of views which will shape these two hallmark social insurance programs for decades to come.

The debate about health care reform has gone on for many years. The debate has generated often innovative ideas for reform that transcend national boundaries. For example, proposals have been made for an extension of health insurance with a framework of social protection; progressive development of funding methods; controls on expenditure; bringing benefits into line with what is actually needed; and the application of technological advances. The book concentrates on health insurance policy innovations in selected countries in Africa, the Americas, Asia, and Europe. In addition this book addresses recent institutional economic findings with regard to application of information technology in health insurance systems. Topics discussed include: new approaches in extending coverage in a health insurance system, confronting resource scarcity: innovative strategies, refining benefits to meet current needs, new institutional and administrative frameworks, and transformation through information technology systems. Many of the innovations presented here have already been integrated into existing reforms and the authors refer to concrete developments in individual countries and regions. This book presents those important innovations to social health insurance systems in industrialized and developing countries that have been brought forward in recent years and, as far as possible, already evaluated. In doing so, the intention is to show developments that are valid for more than one country and could have long term impact on health insurance systems. This book is intended to promote deeper exchanges of experience between differing regions, countries, and health insurance systems. In this way it will stimulate ideas for politicians and practitioners, scientists and other experts.

The reform of social security pensions and healthcare is a key issue for the modern world, and in many ways Latin America has acted as a social laboratory for the reform of these systems. From the reforms that took place in Chile in 1981, most pension and health care systems in the region have seen reform, and been fully or partially privatized. Many other countries considering reform of their own systems have been influenced by the policies implemented in Latin America. Yet despite the importance and influence of these reforms, until now there has not been an integrated and comprehensive analysis of the changes and their effects. This book is the result of four years of painstaking work, data collection, field research and international collaboration, and so fills the vacuum in the literature with a systematic comparison of pension and healthcare reforms in the 20 Latin American countries. It identifies reform models, and elaborates taxonomies to facilitate their understanding and comparison. Some key features of the reforms to emerge are: labour force and population coverage, equity and solidarity, sufficiency and quality of benefits, state regulation, competition and degree of privatization, efficiency and administrative costs, social participation in management, financing sources and long-term sustainability. Effects of the reforms on social security principles are measured based on recent standardized statistics and other information. Goals or assumptions of the reforms are contrasted with actual outcomes, and the pros and cons of private versus private provision assessed. Detailed policy recommendations are offered to correct current problems and improve pension and healthcare systems. This is the first book to comprehensively study these influential reforms in Latin America's pension and health care systems, and as such will be of importance to academics and researchers interested in social security and welfare policy, pensions, health care, and public policy; Social security, pension, and health care policy-makers; And social security, pension, and health care consultants and practitioners. The Social Security Administration (SSA) administers two programs that provide benefits based on disability: the Social Security Disability Insurance (SSDI) program and the Supplemental Security Income (SSI) program. This report analyzes health care utilizations as they relate to impairment severity and SSA's definition of disability. Health Care Utilization as a Proxy in Disability Determination identifies types of utilizations that might be good proxies for "listing-level" severity; that is, what represents an impairment, or combination of impairments, that are severe enough to prevent a person from doing any gainful activity, regardless of age, education, or work experience.
