Surgery for the treatment of deformities such as scoliosis carries a small but significant risk of damage to the spinal cord through inadvertent compression or interference with the blood supply. Electrophysiological techniques, principally those for recording sensory and motor evoked potentials, offer a means of continuously assessing the conduction of nerve signals up and down the spinal cord and hence the possibility of detecting a defect at an early stage, when the cause may be reversible. This form of monitoring is now accepted practice in many countries. The main contributors to the Handbook are surgeons, neurophysiologists and anaesthetists at the forefront of research. From the United States, where practice has been most widespread, come the results of the first large-scale, multi-centre survey into monitoring methods and their effectiveness. During the last decades, research on spinal cord has attracted a great deal of attention because of problems such as sensory-motor and autonomic dysfunctions associated with traumatic and other injuries. Recording of spinal cord evoked potentials is one of the most promising approaches to understand the spinal cord function. However, the details of the evoked potentials and their significance in various pathological conditions are not yet fully characterized. This book summarizes new findings in the field of electrophysiology and relates this knowledge to pathology and regeneration research. The spinal cord has a characteristic structure and functions that are distinct from those of the brain. Its functions are tremendously important since it modulates the peripheral sensory inputs to the dorsal horn, and it gives rise to the ascending pathways transmitting peripheral afferent inputs to the brain, and conveys the descending pathways from the brain both to the lower motor neurons, the final common pathway, and to dorsal horn sensory neurons. In spite of these vital functions, the spinal cord constitutes only a small percent age of the mass of the human central nervous system and is located far from the skin surface, which has obstructed the recording of its electrical activity. Recently, however, important advances have been made in several recording techniques, including epidural recording or averaging methods, allowing both sensory and motor evoked spinal cord potentials in man to be recorded. This volume is based on the papers presented at the Fourth International Symposium on Spinal Cord Monitoring and Electrodiagnosis. Each of these international symposia has brought together many of the specialists involved in this research, with an important increase in the number of participants since the first symposium was held in Toyko in 1981. At the past symposia several attempts were made to standardize data, techniques, and clinical applications and to integrate the new findings into patient care.

The symposium from which this book originates represents a sig nificant watershed in the field of intraoperative neural monitoring, since the participants concluded that electrophysiologic monitoring techniques should be considered a "standard of care" for surgical pro cedures that place the central nervous system (CNS) at risk for injury. Specifically, it was agreed that the somatosensory-evoked potential (SEP) is a remarkably reliable and sensitive indicator of several aspects of CNS function, and should be routinely employed as an intraopera tive monitor during many neurosurgical and orthopedic procedures. The significance of this conclusion cannot be overstated, for at the time of this writing, intraoperative monitoring methods based on evoked-potential analyses are still considered experimental and are not in routine use. The reasons for this are not clear, given the accu mulation of literature and expertise on this subject over the past five years. Granted, the cost of electrophysiological monitoring equip ment is high, but only initially. The benefits of injury prevention far outweigh these costs, from both medical and economic viewpoints. It is our sincere hope and goal that the medical community be made aware of the value of intraoperative neural monitoring.
