Spinal instability is the inability of the various spinal structures to limit displacement under physiological loads in order to prevent neurological damage, incapacitating pain, and major deformity. There is no quantitative gold-standard definition for non-traumatic instability. The study objectives were to quantify unstable motion for the pediatric to elderly spine and to assess instability for ligament injury and multiple laminectomy. The novel instability metric, a collection of instability data, was produced from normal range of motion data and a published guideline for adult spinal instability. A high-fidelity finite element model of the adult lumbar spine was modified to test the sensitivity of singular ligament removal and size and location of multiple laminectomy to induce instability under physiological loads. Range of motion indicating instability increases in pediatric patients until the onset of puberty and then declines with age. Singular ligament removal and one-, two- and three-level laminectomy did not induce instability except in the case of facet capsular ligament removal. It is difficult and expensive to study spinal disorders in vivo making it advantageous to study them ex vivo. The spinal instability metric (SIM) will provide a standard for application with any spine model, clinical or experimental, to assess the risk of developing the instability-induced effects of pain, neurological damage and deformity.
