Social isolation and loneliness are serious yet underappreciated public health risks that affect a significant portion of the older adult population. Approximately one-quarter of community-dwelling Americans aged 65 and older are considered to be socially isolated, and a significant proportion of adults in the United States report feeling lonely. People who are 50 years of age or older are more likely to experience many of the risk factors that can cause or exacerbate social isolation or loneliness, such as living alone, the loss of family or friends, chronic illness, and sensory impairments. Over a life course, social isolation and loneliness may be episodic or chronic, depending upon an individual's circumstances and perceptions. A substantial body of evidence demonstrates that social isolation presents a major risk for premature mortality, comparable to other risk factors such as high blood pressure, smoking, or obesity. As older adults are particularly high-volume and high-frequency users of the health care system, there is an opportunity for health care professionals to identify, prevent, and mitigate the adverse health impacts of social isolation and loneliness in older adults. Social Isolation and Loneliness in Older Adults summarizes the evidence base and explores how social isolation and loneliness affect health and quality of life in adults aged 50 and older, particularly among low income, underserved, and vulnerable populations. This report makes recommendations specifically for clinical settings of health care to identify those who suffer the resultant negative health impacts of social isolation and loneliness and target interventions to improve their social conditions. Social Isolation and Loneliness in Older Adults considers clinical tools and methodologies, better education and training for the health care workforce, and dissemination and implementation that will be important for translating research into practice, especially as the evidence base for effective interventions continues to flourish.

PACE (Program of All-Inclusive Care for the Elderly) is a program designed to provide seniors with comprehensive, coordinated, and community-based care to meet their unique medical needs. One of the most current problems faced by PACE program A is their retention rate. Dissatisfaction with the quality of care is the number one catalyst for the low retention rates, encompassing scheduling of services, transportation services, among many other reasons. The goal of this research is to explore international strategies the PACE program can adapt to improve the current model. Improvements to the PACE model can increase satisfaction of the participants and can lead to higher retention rates and yield more favorable outcomes. The methodology used for this this research is a mixture of qualitative and quantitative methods to provide a comprehensive understanding of the research conducted. As part of the qualitative data collection, interviews were used to assess satisfaction levels and gather suggestions for improvements. For quantitative data collection, enrollment and retention rates were analyzed to understand the retention and satisfaction in meeting the ongoing needs and expectations of the participants. There are some limitations to the research conducted for this project, as similar healthcare models from other countries were used. Cultural and socioeconomic differences may impact the way other countries tailor their models compared to the United States (US). Also, the variation of model structures, services, and eligibility criteria can influence the program's effectiveness and outcomes. The data availability and quality could also affect the accuracy and generalizability of the findings. Following a comprehensive examination of international models similar to the PACE model, there are viable avenues for eliminating some of the issues faced by PACE program A. Despite the existence of various PACE program models in other countries, these programs offer services that can be adapted by PACE program A to help alleviate some of the current challenges. These adaptations include technological and social innovations as well as mediations to their top disenrollment reasons. The Age Well at Home initiative in Canada offers a volunteer-friendly visiting program in the home, which not used in PACE program A. With PACE program A growing at a fast rate, one of the services some of the population served requests is more day center attendance to socialize. The assignment of volunteers designated to make home visits to provide companionship, emotional support, or social interaction would lessen such requests considering there are other participants necessitating day center attendance due to medical conditions such as dementia, depression, etc. Within United Kingdom, the National Health Service (NHS) uses technology to reduce the need for unnecessary in-person appointments. The technological integration not only serves to mitigate some of the current transportation challenges encountered within PACE program A, but also may serve to free up more time for doctors and nurses to tend to more urgent needs of other participants. Additionally, eliminating some of the transportation costs or helping allocate those services towards other transportation needs. Australia uses technology with a different approach. Australia uses "My Health Record", giving patients access to their medical test results, health information, and treatment plans. Having information readily accessible helps decrease the number of appointments for individuals with those preferences which can be reallocated for alternative purposes.
