Micro-trauma: A psychoanalytic understanding of cumulative psychic injury explores the "micro-traumatic" or small, subtle psychic hurts that build up to undermine a person’s sense of self-worth, skewing his or her character and compromising his or her relatedness to others. These injuries amount to what has been previously called "cumulative" or "relational trauma." Until now, psychoanalysis has explained such negative influences in broad strokes, using general concepts like psychosexual urges, narcissistic needs, and separation-individuation aims, among others. Taking a fresh approach, Margaret Crastnopol identifies certain specific patterns of injurious relating that cause damage in predictable ways; she shows how these destructive processes can be identified, stopped in their tracks, and replaced by a healthier way of functioning. Seven different types of micro-trauma, all largely hidden in plain sight, are described in detail, and many others are discussed more briefly. Borderline Personality Disorder (BPD) is a diagnosis given to ten percent of all those seen in outpatient mental health facilities and twenty percent of those seen in inpatient psychiatric units. This is a significant number of people in the Western world. Yet many of the core concepts and symptoms that underlie this diagnosis are questionable. Many of the attitudes and actions of carers are based on assumptions about those with BPD that cry out for analysis, with both cultural and gender norms interacting with clinical diagnosis and treatment, to the detriment of both carers and patients. This book considers how we diagnose BPD, looking at the key constructs: identity disturbance, inappropriate or excessive anger, unstable relationships, impulsivity, self-injurious behaviour, and manipulativity. It starts by looking at the cultural and gender assumptions and norms behind BPD, drawing upon philosophical, clinical, anthropological, and sociological literature. Combining philosophical analysis with clinical experience and patients' writings, it clarifies the constructs so that the reader can understand the messiness and complexity that frames this diagnosis and treatment. After examining the current state of these constructs, and their effects on carer/patient interactions, Part II sees an application of virtue theory to therapeutic treatment with BPD patients. It looks at three virtues that are particularly important for clinicians and other carers to cultivate when working with BPD patients: trustworthiness, the virtue of giving uptake, and empathy. It argues that, in their absence, not only are clinicians' attitudes harmful to patients but that the status of the diagnosis is actually compromised. This book explores the roots of borderline states of mind in early relational trauma and shows how it is possible, and necessary, to visit 'the darkest places' in order to work through these traumas. This is despite the fact that re-experiencing such traumas is unbearable for the patient and they naturally want to enlist the analyst in ensuring that they will never be experienced again. This is the backdrop for the extreme pressures and roles that are constellated in the analysis that can lead to impasse or breakdown of the analytic relationship. The author explores how these areas can be negotiated safely and that, whilst drawing heavily on recent developments in attachment, relational, trauma and infant development theory, an analytic attitude needs to be maintained in order to integrate these experiences and allow the individual to feel, finally, accepted and whole. The book builds on Freud's views of repetition compulsion and re-enactment and develops Jung's concept of the traumatic complex.

Where do spontaneous thoughts come from? It may be surprising that the seemingly straightforward answers "from the mind" or "from the brain" are in fact an incredibly recent understanding of the origins of spontaneous thought. For nearly all of human history, our thoughts - especially the most sudden, insightful, and important - were almost universally ascribed to divine or other external sources. Only in the past few centuries have we truly taken responsibility for their own mental content, and finally localized thought to the central nervous system - laying the foundations for a protoscience of spontaneous thought. But enormous questions still loom: what, exactly, is spontaneous thought? Why does our brain engage in spontaneous forms of thinking, and when is this most likely to occur? And perhaps the question most interesting and accessible from a scientific perspective: how does the brain generate and evaluate its own spontaneous creations? Spontaneous thought includes our daytime fantasies and mind-wandering; the flashes of insight and inspiration familiar to the artist, scientist, and inventor; and the nighttime visions we call dreams. This Handbook brings together views from neuroscience, psychology, philosophy, phenomenology, history, education, contemplative traditions, and clinical practice to begin to address the ubiquitous but poorly understood mental phenomena that we collectively call 'spontaneous thought.'
