The prevalence of substance abuse in the severely mentally ill is higher than that in the general population, making this a serious issue for clinicians. Integrated treatment, although the most widely adopted approach, is subject to tremendous variation in its operationalisation, especially throughout different parts of the world. Substance Misuse in Psychosis presents the latest international developments and practical treatment interventions that can be used with co-morbid individuals and their families. Different social and cultural contexts are described and contrasted, along with treatment approaches that have been tailored to address the needs of the severely mentally ill. A final section considers sub-groups, e.g. the young, the homeless, outlining the special issues that need to be considered when providing services for these groups.

Estimates indicate that as many as 1 in 4 Americans will experience a mental health problem or will misuse alcohol or drugs in their lifetimes. These disorders are among the most highly stigmatized health conditions in the United States, and they remain barriers to full participation in society in areas as basic as education, housing, and employment. Improving the lives of people with mental health and substance abuse disorders has been a priority in the United States for more than 50 years. The Community Mental Health Act of 1963 is considered a major turning point in America's efforts to improve behavioral healthcare. It ushered in an era of optimism and hope and laid the groundwork for the consumer movement and new models of recovery. The consumer movement gave voice to people with mental and substance use disorders and brought their perspectives and experience into national discussions about mental health. However over the same 50-year period, positive change in American public attitudes and beliefs about mental and substance use disorders has lagged behind these advances. Stigma is a complex social phenomenon based on a relationship between an attribute and a stereotype that assigns undesirable labels, qualities, and behaviors to a person with that attribute. Labeled individuals are then socially devalued, which leads to inequality and discrimination. This report contributes to national efforts to understand and change attitudes, beliefs and behaviors that can lead to stigma and discrimination. Changing stigma in a lasting way will require coordinated efforts, which are based on the best possible evidence, supported at the national level with multiyear funding, and planned and implemented by an effective coalition of representative stakeholders. Ending Discrimination Against People with Mental and Substance Use Disorders: The Evidence for Stigma Change explores stigma and discrimination faced by individuals with mental or substance use disorders and recommends effective strategies for reducing stigma and encouraging people to seek treatment and other supportive services. It offers a set of conclusions and recommendations about successful stigma change strategies and the research needed to inform and evaluate these efforts in the United States.

Addiction is increasingly being recognized as a major global public health issue, and an ever-growing number of medical specialties, psychological and social science training programs, and professional associations are including addiction as part of their training and continuing education curricula. The first edition of this book presented an overview of the spectrum of addiction-related problems across different cultures around the globe. Sharing the experience and wisdom of more than 260 leading experts in the field, and promoted by the International Society of Addiction Medicine, it compared and contrasted clinical practices in the field of addiction medicine on the basis of neurobiological similarities as well as epidemiological and socio-cultural differences. Building on the success of this inaugural edition, and taking into account the formal and informal comments received as well as an assessment of current need, this textbook presents general updated information while retaining the most requested sections of the first edition as demonstrated by the number of chapter downloads. It also provides a basic text for those preparing for the ISAM annual certification exam. Written by some 220 international experts, it is a valuable reference resource for anyone interested in medicine, psychology, nursing, and social science.

The Recognition And Management Of Early Psychosis written by Patrick D. McGorry and has been published by Cambridge University Press this book supported file pdf, txt, epub, kindle and other format this book has been release on 1999-07-28 with Psychology categories.

The treatment of early psychosis has been bedevilled by an entrenched pessimism, stemming from the asylum era and the Kraepelinian model of schizophrenia. More recently, however, there has been a surge of interest in preventively oriented treatment of patients showing the first signs of psychotic illness, with the realization that these illnesses are frequently highly responsive to early treatment. This is the first text to focus on the potential of early detection of psychosis, and the practicalities of treatment. Based on the pioneering experience and research of a now well-established prevention and intervention centre, and with contributions from international authorities, the book outlines a framework for intervention, reviews the evidence available to guide clinical practice, and describes models of treatment. Incorporating many personal narratives and case histories, it is strong on theory, sensitive on practical issues, and will challenge, inform and guide clinicians.

This book fills a gap in the existing medical literature by providing a best-practice approach to the evaluation and acute treatment of patients presenting for emergency care with identifiable substance use and/or co-occurring psychiatric disorders. As the first interdisciplinary book to integrate psychiatric and emergency care, the text uniquely covers a myriad of serious medical conditions, acute mental status and dangerous behavioral abnormalities. The book focuses on guidelines that support emergency room physicians with little formal medical training in addiction medicine. The first section focuses on the diagnosis and management of substance-specific intoxication and withdrawal states, as well as common medical co-morbidities and disposition considerations. The book lends particular attention to the identification and stabilization of high risk medical conditions associated with each substance of abuse. The second section is psychiatrically focused, addressing the most common psychiatric symptoms and syndromes, their association with SUDs, an approach to differential diagnosis, and discussion of crucial treatment considerations for both safe ED management and post-ED disposition. A final section includes other pertinent topics, for example, the assessment of patient safety, responding to the medication-seeking patient, assessment and treatment of pregnant patients and working with adolescents and their families around substance use. Substance Use and The Acute Patient is a unique and valuable contribution to the literature for both consulting psychiatrists, emergency medicine specialists, addiction medicine specialists, and all other medical professionals who provide care for these most complex and underserved patients.
