This report presents the latest updates related to food security and nutrition in Europe and Central Asia, including estimates on the cost and affordability of healthy diets. It also explores how governments are supporting the food and agriculture sector and how to repurpose policies and incentives to make healthy diets more affordable and agrifood systems more environmentally sustainable. The new estimates confirm that the prevalence of hunger at chronic or severe levels is relatively low in the ECA region, through the prevalence of food insecurity at moderate or severe levels can be quite high. The region is seeing alarmingly high – and rising – rates of overweight and obesity. The COVID-19 pandemic has added 25.5 million people in the region to the ranks of the moderately or severely food insecure, leaving them without access to safe, nutritious and adequate food. The war in Ukraine has made the situation worse. Almost all ECA subregions are experiencing increased costs and reduced affordability of healthy diets because of higher food prices and lower incomes. This report contains an in-depth analysis of the repurposing of food and agricultural policies to ensure the food systems transformation is better suited to addressing the “triple challenge” of achieving food security and good nutrition for better health, providing livelihoods to farmers and others connected to the sector, and reducing the nature and climate footprint of the sector. This report also reviews complementing policies within and outside of agrifood systems to assess whether repurposing efforts are impactful in the ECA region.

Investments in social assistance programs (SAPs) in low- and middle-income countries (LMICs) are increasing. As investments increase, the objectives of these programs are expanding from focusing on reducing poverty to addressing other social issues such as improving diets and nutrition. At the same time, there is increasing interest in addressing all forms of malnutrition within the framework of food systems. Given the intersections between SAPs and food systems, we reviewed the effectiveness of SAPs (agriculture asset transfers, cash transfers, in-kind transfers, vouchers, public works and school meals programs) for reducing all forms of malnutrition across the lifecycle within a food systems framework. As several programs included multiple treatment arms, each representing a unique program design, we used study arm as the unit of analysis and assessed the proportion of study arms with positive or negative program impacts on diet and nutrition outcomes among men, women and children. The majority of the studies included in this review were from evaluations of agriculture asset, cash and in-kind transfer programs. There was clear evidence of positive impacts on women’s and children’s diet-related outcomes. Very few studies assessed program impact on women’s nutritional status outcomes. However, there was some evidence of impacts on increasing body mass index and hemoglobin concentration (Hb) with in-kind transfer programs. Among children, several study arms across the agriculture asset, cash and in-kind transfer programs found positive impacts on increasing height-for-age Z-score (33%-45% of study arms) and weight-for-height Z-score (33%-50% of study arms) and decreasing the prevalence of wasting (43%-60% of study arms). Cash and in-kind transfer programs also found positive effects on reducing stunting prevalence in 33% and 45% of study arms, respectively. Lastly, a few study arms assessed program impact on increasing Hb with some evidence of positive impacts in in-kind and school feeding programs. There was a paucity of relevant evidence of the effectiveness of voucher and public works programs on diet and nutrition outcomes, for men’s outcomes and on micronutrient status. Several challenges remain in understanding the potential for SAPs to improve diet and nutrition outcomes within food systems including the heterogeneity of program and evaluation designs, populations targeted by the programs and included in evaluations and indicators used to assess impact. Addressing these challenges in future evaluations is important for informing program and policy actions to improve the effectiveness of SAPs within food systems for improving diet and nutrition outcomes across the lifecycle.

The Regional Overview of Food Security and Nutrition in Europe and Central Asia for 2023 – Statistics and Trends provides a comprehensive analysis of Sustainable Development Goal 2, focusing specifically on Target 2.1 (ending hunger and ensuring universal access to food) and Target 2.2 (eradicating all forms of malnutrition). Additionally, the report evaluates progress concerning three global nutrition targets: adult obesity, exclusive breastfeeding, and low birthweight, as endorsed by the 2012 World Health Assembly. It also offers an updated analysis of the cost and affordability of a healthy diet within the Europe and Central Asia region. Recent estimates affirm that hunger prevalence remains relatively low in the ECA region. Food insecurity at moderate or severe levels is notably lower compared to global estimates. However, food insecurity levels remain significantly higher than those recorded before the COVID-19 pandemic. Although the ECA region has made significant strides in reducing undernutrition overall, some countries still exhibit relatively high rates of stunting (over 10 percent) and wasting (over 3 percent). Overall, the region is not on track in addressing childhood overweight, adult obesity, anemia among women aged 15 to 49, and exclusive breastfeeding. Healthy diets play a crucial role in safeguarding against the impacts of malnutrition, fostering improved health outcomes. Notably, in the past year, the Western Balkans experienced the highest cost for a healthy diet within the region, surpassing both the ECA and global averages. Overall, there has been an increase in the number of individuals able to afford a healthy diet over the past year, while the percentage unable to afford it remains significantly lower than the global estimate.

Malnutrition in all its forms is a major challenge everywhere in the world, and particularly in low and middle income countries. The briefs have been prepared on the basis of a joint symposium and are intended to give policymakers and program designers in the areas of social protection, nutrition, agricultural policy, and the digitalization of agriculture a quick overview of research-based recommendations on key policy issues that will better enable Egypt to achieve several of the goals outlined in the Sustainable Development Strategy 2030. Addressing all forms of malnutrition – both undernutrition and overnutrition – remains a human and economic development challenge that Egypt must overcome. This brief highlights three areas of action to significantly improve the nutritional status of all Egyptians: Make domestic food policies more nutrition-sensitive and incentivize food producers and marketers to shift to the production, processing, and marketing of healthier foods. Expand ongoing awareness and educational campaigns, such as the 100 Million Healthy Lives initiative, to promote improved nutrition and to disseminate nutrition knowledge. The COVID-19 pandemic is an additional reminder and rationale for investing in healthy diets and sustainable food systems. Promote research to make nutrition policy more effective and efficient, including by increasing collection of nutrition-related data and better sharing of already collected data among ministries, research institutes, and other stakeholders.
