Diabetes in Qatar has seemingly become an enormous challenge to public healthcare systems. Qatar is one of the GCC countries which are known to have the highest diabetes incidence rates. In a global ranking, Qatar is ranked six whereas the United Arab Emirates and Saudi Arabia who are members of the GCC countries are ranked second and third, respectively in regard to incidence rates of diabetes. Current epidemiological reports indicate that an estimated 16 percent of the adult population in Qatar is suffering from diabetes, in which type 2 diabetes is the most prevalent compared to type 1 diabetes. However, other clinical reports indicate that diabetes prevalence among children below the age of five years in Qatar has also assumed upward trends. For instance, prevalence rate for diabetes among children was reported to be 13.7% in 1997, but this rate has more than doubled to reach 28.2% in 2007. From an epidemiological perspective, the burden of diabetes to Qatar’s healthcare system seems to be overwhelming. Therefore, it has become a significant concern in community health nursing because the number of Qatari diabetic patients seeking for medical treatment has increased significantly. This situation has been worsened by the rising trends of obesity prevalence rates. Currently, 45% of the population in Qatar is obese in which 55% of diabetic patients are obese. Therefore, this correlation in incidence and prevalence rates of the two diseases complicates prevention and health promotion measures in Qatar. As a result, Qatar’s efforts to reduce the prevalence of diabetes have been faced with enormous challenges. However, the country is optimistic that the Qatar Diabetes Project and Stem Cell Projects will be successful. In regard to the risk factors, the unprecedented increase of diabetes prevalence is attributable to lifestyle and genetic factors. Clinical studies reveal that most people in Qatar have changed their cultural lifestyle to western-related lifestyle, and this is believed to have contributed significantly to the current diabetes crisis. In general, diabetes issue in Qatar needs appropriate epidemiological measures to reduce its burden to the country’s healthcare system in which social health determinants and epidemiological indicators will help in addressing the issue.

Background:Recent decades have witnessed dramatic surges in the prevalence of non-communicable diseases (NCDs) such as diabetes mellitus (DM), particularly in low- and middle-income countries. Unfortunately, diagnosis, treatment, and control of DM often remain elusive, and complications common. One potential strategy, successfully used for HIV and TB, is the development of integrated care models that empower and capacitate front-line health workers (FLHWs).Aims:HealthRise, a global project focused on expanding access to care for underserved populations, has implemented community-based interventions in four countries with large and growing burdens of DM: Brazil, India, South Africa, and the United States. The project aims to strengthen health systems by improving the capacity of FLHWs and increasing coordination between primary health care and community services. A comprehensive monitoring and evaluation framework was established to measure the interventionsu2019 impacts on rates of disease diagnosis, treatment, and control. Needs assessments were conducted as a baseline for the impact evaluation; baseline results are presented here.Methods:Within each country, diverse sub-geographies representing high or increasing burden of DM were selected through local collaboration. Needs assessments were designed to assess supply- and demand-side barriers influencing the continuum of care from diagnosis, to treatment and control. Mixed methods were applied utilizing primary and secondary data sources; the former largely in India and South Africa, the latter in Brazil and the United States. Results are presented descriptively.Results:The prevalence of DM ranged from 5-9% across all sites. The prevalence of DM risk factors among the general population was high in all countries, with the highest rates of hypertension and elevated BMIs in South Africa. More than 70% of individuals with DM reported a diagnosis, although many patients reported seeking care only after developing symptoms. Once diagnosed, patients were likely to initiate treatment, but successful management of conditions was challenging even in the highest-income populations. Qualitative data indicated that access to care was often challenging, particularly in poorer communities where travel time and costs posed significant barriers. Furthermore, patients reported difficulties managing medications, adjusting behaviors, and monitoring their condition. This was compounded by either a lack of community-based resources or poor coordination between these services and the health care system.Discussion:The overwhelming majority of patients with DM were aware of their disease status, and most initiated pharmacologic treatment. However, many patients faced barriers to continuous access, resulting in inadequate disease control. Our results suggest patients need more accessible and frequent support maintaining healthy lifestyles, maintaining treatment regimens, monitoring conditions, and adjusting treatment as necessary. HealthRise has launched interventions to address these gaps and improve outcomes by building community health worker capacity, integrating their work into the formal health care system, and empowering patients with support systems. Results of the final impact evaluation will provide further evidence on how community-based strategies can improve DM care across a wide range of settings.

Chronic conditions and diseases are the leading cause of mortality and morbidity in Europe, accounting for 86% of total premature deaths, and research suggests that complex conditions such as diabetes and depression will impose an even greater health burden in the future - and not only for the rich and elderly in high-income countries, but increasingly for the poor as well as low- and middle-income countries. The epidemiologic and economic analyses in the first part of the book suggest that policy-makers should make chronic disease a priority. This book highlights the issues and focuses on the strategies and interventions that policy-makers have at their disposal to tackle this increasing challenge. Strategic discussed in the second part of this volume include (1) prevention and early detection, (2) new provider qualifications (e.g. nurse practitioners) and settings, (3) disease management programmes and (4) integrated care models. The foundations of modern physical anthropology, like those of modern biology, rest on neo-Darwinian theory, which provides the common background for a diverse discipline that lacks a common set of methods. Physical anthropologists recognize that the interaction between human culture and biology has shaped and maintained our species, and that full understanding of the processes responsible requires consideration of both biology and culture. A large number of studies have been conducted in different parts of the world, including India to find out the variation of the genetic markers in human blood, viz., (1) blood group polymorphisms: the ABO system, the Rh (D)system, the MNSs system, the Duffy system, the Lutheran system, the Kell system, the Kidd system, secretor and non-secretor system; (2) serum protein polymorphisms: haptoglobin system, transferring system, group specific component system, etc.; (3) enzyme polymorphism: red cell acid phosphatase system, phosphoglucomutase system, 6- phospogluconate dehydrogenase system, glucose-6-phosphate dehydrogenase system, etc.; and (4) blood cell proteins: haemoglobin variants.
