This edited volume makes the case that a systems approach is needed for long-term spaceflight missions. Due to distance from Earth and the impossibility of resupply or rapid return, these flights will entail unprecedented autonomy on the part of the crew. This autonomy includes the need for local clinical decision support and other assistance with urgent medical concerns. The advantage of a systems approach to medical needs in human spaceflight is that it enables a systematic and repeatable methodology for identifying and developing the individual pieces of the spacecraft systems based on the human system needs that are unique to that problem and environment.The book begins by introducing a systems basis for thinking about humans in space. It describes existing models and paradigms of systems medicine, as well as the tools and analytical approaches involved. A tantalizing peek at preliminary applications of systems medicine in NASA is provided. As systems in human spaceflight continue to evolve for the foreseeable future, it is argued that a systems approach will help to ensure that the evolution of the relevant sub-systems is centered on optimal human-system function across the unique spaceflight needs of the developing customer base. The discussions on balancing the needs of the mission versus the maintenance of crew health, medical risk and liability for commercial spaceflights, and the need to anticipate what the next medical issues might be, will hopefully leave readers cautiously optimistic about this new frontier of applied science.

Investigations in space have led to fundamental discoveries of the human body to the space environment. Gilles Clément has conducted extensive research in this field. This readable text presents the findings from the life science experiments conducted during and after space missions. About 1200 human space flights have been completed to date, including more than 500 astronauts from various countries, for a combined total presence in space of about 90 years. The first edition of this title was published in 2005 (written in 2003 – 2004), and new data is now available from crewmembers participating in long-duration flights on board the International Space Station (ISS). The number of astronauts who have spent six months in orbit has doubled since 2004. On board the ISS, the astronauts use newly developed pharmaceutical countermeasure for bone loss (such as biophosphonates) and state-of-the-art exercise resistive devices against muscle atrophy and cardiovascular deterioration. Many breakthroughs in biological research and translational healthcare advancements have been achieved by studying the response of biological systems to extreme environments. The spaceflight platform provides a unique environment where researchers can explore fundamental questions into cellular and molecular response mechanisms to unveil novel insight into human health and disease. The 1998 Committee on Space Biology and Medicine (CSBM) report A Strategy for Research in Space Biology and Medicine in the New Century assessed the known and potential effects of spaceflight on biological systems in general and on human physiology, behavior, and performance in particular, and recommended directions for research sponsored over the next decade by the National Aeronautics and Space Administration (NASA). The present follow-up report reviews specifically the overall content of the biomedical research programs supported by NASA in order to assess the extent to which current programs are consistent with recommendations of the Strategy report for biomedical research activities. In general, NASA programs concerned with fundamental gravitational biology are not considered here. The committee also notes that this report does not include an evaluation of NASA's response to the Strategy report, which had only recently been released at the initiation of this study. Review of NASA's Biomedical Research Program summarizes the committee's findings from its review of (1) NASA's biomedical research and (2) programmatic issues described in the Strategy report that are relevant to NASA's ability to implement research recommendations.

This series of reports deals with ongoing research by NASA into a risk reduction strategy for human space exploration and planned manned missions to Mars. Each report deals with a unique aspect of the human research needed to pursue long-duration missions. However, current studies onboard the International Space Station (ISS) that are defining space normal for the human immune system remain insufficient to determine clinical risk during exploration space missions. In particular, more in-flight studies are needed using human subjects. Crew health and performance is critical to successful human exploration beyond low Earth orbit. The Human Research Program (HRP) investigates and mitigates the highest risks to human health and performance, providing essential countermeasures and technologies for human space exploration. Risks include physiological and performance effects from hazards such as radiation, altered gravity, and hostile environments, as well as unique challenges in medical support, human factors, and behavioral health support. The HRP utilizes an Integrated Research Plan (IRP) to identify the approach and research activities planned to address these risks, which are assigned to specific Elements within the program. The HRP utilizes various research platforms to conduct research. Ground research occurs in laboratories and analogs that mimic a portion of the spaceflight environment.
