I have been asked to write a foreword to the next edition of the Vademecum of Clinical Bio chemistry renamed to Differential Diagnosis by Laboratory Medicine. The Editor in-Chief, Professor Dusan Mesko, conceived the idea of the Vademecum during a very intensely, in tellectually active period as a visiting Olga Havel Fellow from Slovak Republic at the Mayo Clinic in Rochester. It was here during his study stay that much of the conceptual planning and detailed realization was being completed. It was with much interest that I observed Professor Mesko in his effort, and I know while here he has gained admiration of those who had the privilege of interacting with him. When the first edition of the Vademecum ap peared and we received the copy for the Mayo Library and for myself, I was overcome with a genuine sense of joy. In this era of rapid information and the need to access usable infor mation book such as this cannot be but judged extremely useful. It is my hope that this work, which attests to the thoroughness of Professor Mesko's and his co-workers efforts will prove to be fruitful for the students quick reference, for clinicans and house officers lightening of burdens, particularly of on-call nights, and for the teaching staff as well to quickly access needed information. With these sentiments, we wish the authors and the book well and also hope that the potential widening circle of readers and users will benefit from the work pre sented.

and User's Guide Abdominal ultrasound examinations are now performed mainly with real-time instrumentation, thereby giving the user tremendous flexi bility in the manner in which the examination is conducted. However, with such flexibility come significant problems as well. Because fields of view are small, and because the operator can readily move the probe anywhere in the abdomen to see structures in a variety of planes and sections, it is relatively easy to become disoriented or to miss significant findings unless the examination protocol is care fully devised. The operator must approach the task of scanning the patient in a logical and organized way that is related to the patient's clinical findings as well as to the findings discovered during the ultra sound examination. This book has been written to assist the operator in performing an organized and directed ultrasound examination by providing in outline form a sequential approach to the scanning' of abdominal organs and regions. This approach has several functions: 1) to indicate the structures within the organ or region that should be scanned; 2) to suggest other regions to examine if abnormalities within the initially imaged structures are detected; and 3) to suggest differential diagnostic possibilities when abnormalities are seen.

Adult neuropsychiatry is now a well-established field with numerous reputable references. Practitioners who work with children routinely note how references and practitioners knowledgeable in the equivalent work in the pediatric world are rare. Child psychiatrists and neurologists frequently work with individuals struggling with these conditions and would strongly benefit from such a reference that incorporates medical work-up, psychopharmacological recommendations, family/support recommendations and theoretical pathophysiology. Pediatricians and developmental pediatricians often treat children with behavioral and neuropsychiatric sequelae, but are not well-trained in the neuropsychiatric management of these cases. Neuropsychologists and educational psychologists working with children and adults with pediatric-onset conditions will also find the text helpful to contextualize their cases, better-understand the medical evaluation and management and perhaps adjust recommendations that would supplement their own testing methods. Finally, sub-specialists in adult neurology, psychiatry and neuropsychiatry often find themselves working with these children by default as there are few pediatric subspecialists who are available to accept them into practice. When facing complex neuropsychiatric illness in children, many clinicians are stymied because they may have “never seen a case like that”.
