This study evaluated the interpersonal model of binge eating and associated psychopathology over the course and 4-month follow up of two psychotherapy treatments (Cognitive Behavioral Therapy and Interpersonal Psychotherapy), and examined if treatment effects emerged. The model posits that interpersonal problems lead to negative affect which results in binge eating. The model was tested using data from a clinical sample of 159 women and men with binge eating disorder (BED) who were participants in a comparative psychotherapy treatment trial (Wilfley et al., 2002). Analyses examined a series of longitudinal mediation analyses examining regression pathways, direct effects, and indirect effects. The results did not provide support that psychotherapy treatment addresses the mechanisms of the interpersonal model of binge eating as theoretically proposed. Findings suggest that reductions in interpersonal problems and negative affect over the course of treatment did not differ between CBT and IPT. It is possible that the mechanisms of the interpersonal model of binge eating change on a more momentary level over the course of treatment, and therefore, the constructs in the current study were not measured to be reflective of the potential momentary relations of the theoretical model. Future research should use ecological momentary assessment to explore the interpersonal model of binge eating on a momentary level over the course of psychotherapy treatment.

[Truncated abstract] Bulimia nervosa (BN) is characterised by repeated episodes of objective binge eating and the use of compensatory behaviours intended to prevent weight gain following binge eating episodes. BN is most commonly observed in females, and estimates of the lifetime prevalence in this population range from 1.5 to 2.9%. Cognitive-behavioural therapy (CBT) is the treatment of choice for BN, yet only 30 to 50% of patients experience remission of binge eating and purging following CBT. Improved psychological treatments for BN are therefore needed. One way to improve psychological treatments for BN is to refine our understanding of the mechanisms that maintain this disorder. The cognitive-behavioural theory of BN describes the processes involved in the maintenance of this disorder (Fairburn, Cooper, & Cooper, 1986). This theory was recently enhanced to include four additional maintaining mechanisms: mood intolerance, interpersonal problems, perfectionism, and low self esteem (Fairburn, Cooper, & Shafran, 2003). The primary aim of this thesis was to evaluate the original and enhanced cognitive-behavioural models of BN. This was achieved in three stages. First, three preliminary studies investigated the measurement of mood intolerance, interpersonal problems and perfectionism, and their relationship with eating disorder symptoms, in preparation for subsequent model testing. Second, the original and enhanced cognitive-behavioural models were compared in a clinical sample to determine if the enhanced model accounted for a greater proportion of variance in dietary restraint, binge eating, and purging than the original model. In a series of four studies, the aim of the current research project was to evaluate the cognitive-behavioral theory and treatment (CBT) of eating disorders. The first study (Chapter 5) was a meta-analysis (Linardon, Wade, De la Piedad Garcia, & Brennan, in press) of randomized controlled trials (RCT) testing the efficacy of CBT for eating disorders. Pooling data from 79 RCTs, results showed that therapist-led and guided self-help CBT were efficacious for individuals with bulimia nervosa (BN) and binge eating disorder (BED). There was no evidence to suggest that CBT was more efficacious than other psychological interventions in anorexia nervosa (AN). Having found evidence supporting the efficacy of CBT for certain eating disorder presentations, the second study (Chapter 6), which was a systematic review, focused on identifying the reliable factors that mediate, moderate, or predict outcome during CBT (Linardon, de la Piedad Garcia, & Brennan, 2016b). This review found that mediators and moderators of change have been largely unexplored, and that no reliable predictors of outcome emerged. Therefore, based on existing evidence, it was concluded that it remains unclear how, for whom, and under what conditions, CBT for eating disorders works. To understand the mechanisms through which CBT for eating disorders may work, a cross-sectional evaluation of the cognitive-behavioral model in a large non-clinical sample was employed for the third study (Chapter 8; under review). This study validated the conceptual pathways hypothesized by the cognitive model; it also identified two additional variables that might be important mechanisms of change during CBT, body checking and dichotomous thinking. The inclusion of body checking and dichotomous thinking within the cognitive-behavioral model explained nearly three times the amount of variance in disordered eating symptoms than the model without these variables. The third study offered the necessary statistical support for the cognitive-behavioral model and its hypotheses. The final study (Chapter 10) utilized a single case experimental design (n=8) to test the hypothesized cognitive-behavioral mechanisms of change during a CBT guided self-help program. The potential mechanisms of change examined were shape and weight concerns, dietary restraint, and adherence to regular eating strategies. Preliminary evidence showed that an adherence to regular eating in the second week of CBT was associated with a concurrent decrease in dietary restraint and binge eating. From this study, there was no evidence that other potential mechanisms were operating to reduce binge eating behavior. Based on these four independent research studies, this thesis gathered converging evidence in support for the underling cognitive-behavioral model of eating disorders. Further, these findings suggest that CBT for eating disorders is likely to contain several theory-specific mechanisms that are responsible for this treatments effectiveness. More broadly, the current thesis offers support for the contention that CBT for eating disorders “works” because of the reason outlined by its underlying model and because of its specific therapeutic mechanisms.

ABSTRACT: Heatherton and Baumeister (1991) proposed that individuals engage in binge eating in an attempt to escape aversive self-awareness (escape theory). The current study sought to test the escape theory prediction that individuals who experience a stressor and blame themselves will consume more food than individuals experience a stressor and blame another person. This hypothesis was tested in an experimental study among 155 female undergraduate students who experienced a stressor and were randomly assigned to one of three groups: an internal attribution group (blame stressor on self), external attribution group (blame stressor on another participant), and a control group (no one to blame the stressor on). Following the stressor, participants were given access to chocolate, and told to consume as much as they needed to complete a bogus taste test. Contrary to escape theory predictions, there were no statistically significant differences between groups in the amount of chocolate consumed. In an adequately powered study, the findings did not conform to escape theory prediction. Limitations of the study include use of a nonclinical population and access to only one type of food (chocolate).
