Excerpt from The American Hospital of the Twentieth Century: A Treatise on the Development of Medical Institutions, Both in Europe and in America, Since the Beginning of the Present Century It is in response to this demand that the writer has endeavored to collect plans and information concerning a few of the many good institutions recently finished or under construction, with the hope that interest in the publication of such works will grow and that this book will be only a forerunner of much more comprehensive treatises. It is not the writer's intention to criticise the plans of the institutions here shown, but to present them as various solutions of the great problems of housing and caring for the sick and to point out a few of the findings of his own experience in.the planning of more than fourscore hospitals and institutions. The field is so broad that it is impossible more than to touch upon the various points. If frequent mention is made of hospitals in Europe, it is for the purpose of comparison, with the hope that the study and comparison may interest the reader, as it did the writer in collecting the data. The chapters on the Ward Unit, the Surgical Unit, the Medical Unit, and the Equipment are taken largely from papers by the writer which were read before the American Medical Association and the American Hospital Association. How the evolving concept of chronic disease has affected patients and politics in the United States and Europe. Long and recurring illnesses have burdened sick people and their doctors since ancient times, but until recently the concept of "chronic disease" had limited significance. Even lingering diseases like tuberculosis, a leading cause of mortality, did not inspire dedicated public health activities until the later decades of the nineteenth century, when it became understood as a treatable infectious disease. Historian of medicine George Weisz analyzes why the idea of chronic disease assumed critical importance in the twentieth century and how it acquired new meaning as one of the most serious problems facing national healthcare systems. Chronic Disease in the Twentieth Century challenges the conventional wisdom that the concept of chronic disease emerged because medicine's ability to cure infectious disease led to changing patterns of disease. Instead, it suggests, the concept was constructed and has evolved to serve a variety of political and social purposes. How and why the concept developed differently in the United States, the United Kingdom, and France are central concerns of this work. In the United States, anxiety about chronic disease spread early in the twentieth century and was transformed in the 1950s and 1960s into a national crisis that helped shape healthcare reform. In the United Kingdom, the concept emerged only after World War II, was associated almost exclusively with proper medical care for the elderly population, and became closely linked to the development of geriatrics as a specialty. In France, the problems of elderly and infirm people were handled as technical and administrative matters until the 1950s and 1960s, when medical treatment of elderly people emerged as a subset of their wider social marginality. While an international consensus now exists regarding a chronic disease crisis that demands better forms of disease management, the different paths taken by these countries during the twentieth century continue to exert profound influence. This book seeks to explain why, among the innumerable problems faced by societies, some problems in some places become viewed as critical public issues that shape health policy.

To what extent was the use of new machines influenced by the ideas of scientific medicine and to what extent by the availability of newly structured facilities and trained personnel? Drawing on the medical treatment of more than 2,000 patients in Pennsylvania and New York hospitals, Howell traces the ways in which medical technology was used, not merely how it was talked about. He utilizes a wide range of sources—including medical texts, popular literature, and the visual arts—to explore how technology came to be such a central feature of medical care. Howell also shows how changes in medical practice raised issues of gender, culture, and economics. Howell's analysis is especially timely in light of the ongoing debate over U.S. health care reform, a debate in which a central topic is the use and expense of medical technology. In a concluding chapter he applies the book's historical insights to medical practice today—asking why, for example, modern diagnostic tests have not been used to give doctors more time to spend with patients.

Atlanta is often cited as a prime example of a progressive New South metropolis in which blacks and whites have forged "a city too busy to hate." But Ronald Bayor argues that the city continues to bear the indelible mark of racial bias. Offering the first comprehensive history of Atlanta race relations, he discusses the impact of race on the physical and institutional development of the city from the end of the Civil War through the mayorship of Andrew Young in the 1980s. Bayor shows the extent of inequality, investigates the gap between rhetoric and reality, and presents a fresh analysis of the legacy of segregation and race relations for the American urban environment. Bayor explores frequently ignored public policy issues through the lens of race--including hospital care, highway placement and development, police and fire services, schools, and park use, as well as housing patterns and employment. He finds that racial concerns profoundly shaped Atlanta, as they did other American cities. Drawing on oral interviews and written records, Bayor traces how Atlanta's black leaders and their community have responded to the impact of race on local urban development. By bringing long-term urban development into a discussion of race, Bayor provides an element missing in usual analyses of cities and race relations.
